I provided psychosocial assessments, long range planning and community resource linkage for my patients.

I assisted with advance directives and preliminary funeral planning. I offered direct support to both patients and care givers. I trained staff on Dementia I and II levels. I trained and audited social work staff on a state level. I provided social services related to promoting quality of care and comfort for patients with terminal illnesses.

January 2007-January, 2011, Harmony Care, worked as a hospice social worker and bereavement coordinator.

Clinical Counselor: provider of psychosocial assessments (individual and family therapy), crisis intervention, and support on an outpatient basis. Counseling services at Camp Sandhills and Camp Bennettsville under the umbrella of Caresouth Carolina. I provided services to the geriatric population in two local long-term care facilities until 2007. These services included psycho-social assessments and individual therapy. Many interventions such as reminiscent therapy, music therapy, and bereavement therapies were utilized as well as family support services.
