Provide in the field services as follows: Make home visits to initiate the enrollment process for newly eligible enrollees; locate existing members as part of the member monitoring process; educates prospective and existing members and families on services and benefits HSCSN; assist with the HSCSN Family Support Network, Youth Athletic Program, and other services as HSCSN representative at health fairs, collaborative/community organization events and meetings. 2011-2015 Community Support/Case Manager, First home Care

Assess and develop treatment plans for assigned cases in conjunction with the Program Manager and clinical team members. Serve as a member of the clinical team. Provide community support services to the individual or groups. Monitor services received, and response to treatment. Coordinate community services such as making face-to face contact with the consumer weekly, and collateral contact once a month. Complete CALOCUS and GAINS assessments as required. Display knowledge of interventions and implement them in accordance with the treatment plan and with the fidelity of the practice principals. Assist the families and consumer with accessing community resources, agencies and organizations to meet treatment goals. Complete and submit documentation in accordance with specific deadlines, including but not limited to progress notes, monthly summaries, discharge documents, incident reports and service referrals. Ensure completion of annual assessment of caseload. Attend team and supervision meetings to address the consumer’s needs. Facilitate Team meetings as required.

2009-2011 Day Program Supervisor, Mentor Maryland

Responsibilities include: Supervision and daily oversight of staff and consumers which includes direction of personnel and youth programming functions. Develop and implement all youth programming that promotes the highest level of skill acquisitions and independence for each youth. Promote and market programs to funding sources, community agencies, families, and related groups. Ensure that all records are timely, up-to-date, and in compliance with legal and corporate requirements. Order needed work, program supplies, and other equipment for consumer programming.

Provide service coordination to youth placed at Mentor Maryland. Services included but are not limited to: Assisting Clinical Therapist in treatment assessments of the youth’s and families needs in progress towards specific treatment goals. Maintain collateral contact with the youth’s referring agency, school therapist and other service providers. Provide services to address independent living skills as it pertains to the Independent Living Program. Provide crisis prevention to youth during regular business hours. Provide on-call service on a rotating basis during hours when the office is closed. Provide transportation to youth and families as needed to facilitate in the treatment process. Participate as a Treatment team member in meetings and conferences.
