I am 35 years with good mental and health physic condition and recently I worked surveillance of DSLNG Condensate Exporting Facility Phase 2 Project with a target of no accidents, no harm to people and no damage to the environment. My main responsibility is giving quality services with practical and HSE advices. I have almost 15 years experiences in HSE for oil & Gas sector both onshore & offshore (safety & medic officer). The HSE aspect includes provide HSE program, hazard identification & risk assessment, incident investigation, audit, training/course. Also implement a permit to work system, waste management, emergency response, clinical/medical services, safety drill/exercise, gas testing and monitoring to workplace environment. MAIN RESPONSIBILTY

Prepare HSE routine program, objective and target in related with Key Performance Indicator.
Responsible for monitoring the achievement of company & client HSE objectives & target.
Implement Integrated Management System (ISO 14001 & OHSAS 18001) & oversee the application of company policies.
Evaluate hazard and risk associated to any site work activities and implement mitigation action to eliminate or reduce residual risk to an acceptable level.

Collaborate with engineers and physicians to institute control and remedial measures for hazardous and potentially hazardous conditions or equipment.
Maintain accurate safety register, statistic and HSE reporting/documentation.
Appoint appropriate personnel to undertake safety duties in accordance with safety plan.
Assisting company in implementing of emergency response plan & develop a comprehensive emergency response plan as part of an overall/contingency plan.
To enforce safety requirement by observing and enforcing the rules and procedures.
Ensure all activities covered by permit to work.
To ensure the implementation of corrective actions are carried out consistently by their respective Department / Management.
