WORK HISTORY:
Qualified Intellectual Disability Professional (QIDP): Koinonia Homes Inc,
Independence, OH. (5/3/10 - 4/16/16) My duties include writing Annual
Habilitation Plans (IHPs) and Behavior Support Plans; writing programs that
the clients practice to learn new skills, along with training the direct
care staff on how each program is to be done. Other duties included
gathering monthly documentation, analyzing and recording the results, and
making adjustments as needed. Professional communication with
psychiatrists, psychologists, medical professionals, vocational training
professionals and the agency's Human Rights Committee. I am in a
supervisory capacity in three assigned ICF/DD Homes.
Care Manager: Sunrise Senior Living, Parma, OH. (10/3/11 - current) Part-
time, Duties include direct care of the individuals who live in the
assisted living and the dementia units with their activities of daily
living, build meaningful relationships with seniors and their families,
attend to individual care needs and learn their unique preferences and
personalities. Communication and documentation to nursing and
administrative staff.
Foster Care Case Manager: Bellewood Presbyterian Therapeutic Foster Homes,
Bowling Green, Ky. (11/24/08-10/20/2009) Responsibilities consisted of
intake of new clients (foster children) for placement, monthly contact with
foster children and foster parents, problem solving, and assistance in
client adjustment to State care. Worked in coordination with Bellewood's
Therapist in managing behaviors, scheduling appointments, and follow
through of Behavior Plans of clients placed in foster homes. Contact with
State social workers to help meet the needs of clients on an as-needed
basis. Other duties included monthly documentation, upkeep of client files,
writing Individual Treatment Plans (ITPs), and the transportation of
clients for needed services. Responsible for sharing weekend on-call
services, on a rotation basis.
Reminiscence Coordinator, Assisted Living Coordinator: Sunrise Senior
Living, Brentwood, Tn.; Jenkintown, Pa. (1/22/07-12/23/07) Duties included
supervision of care for residents in the assisted living and dementia
units. Position involved social work responsibilities, direct care
monitoring, and staff training and supervision to meet resident and
budgeting needs. Planned, organized, and conducted support groups as well
as other family-oriented activities designed to enhance residents' care.
Also responsible for writing and updating Individual Support Plans (ISP)
that reflected the residents' abilities and identified areas of needed
skill training for continued growth and rehabilitation. I was the primary
contact for families, hospice, and OT, PT, SLP services. I have a clear
understanding of the expectations of quality control as well as state and
federal documentation requirements.
Residential Program Manager: Region Ten CSB, Charlottesville, Va.-Crozet,
Va. (6/16/04-4/15/06) My responsibilities included social work interactions
with residents and the supervision of daily activities for two
intellectually-disabled adult group homes. Responsible for shift scheduling
and writing evaluations of work performance for those I supervised.
Responsibilities included reporting to two regional directors, providing
verbal and written reports on client progress as well as staff interaction
and conduct. Regular contact with social service agencies, physicians,
families, and other agencies, including those involved with providing day
placements for our clients.
Director of Social Services: Medical Facilities of America, Roanoke, Va.-
Louisa Healthcare (1/14/03-6/1/04) Duties included contact with residents
for information gathering and problem solving in a social work capacity. I
was the primary contact for resident progress and/or discharge. Facilitated
meetings with families and community resources to meet residents' needs.
Forms I am familiar with include Re-certification for Long-term Care, Food
Stamp applications, Medicaid applications, and Payee Reports for the
Department of Social Security. Responsibilities included record keeping and
report writing to remain in compliance with state and federal laws.
Residential Program Manager: Community Residence, Arlington, Va.-Manassas,
Va. (10/01/01-9/27/02) Duties included the supervision of full-time and
relief employees for two Waiver group homes for adults with dual-diagnoses
of intellectual disability accompanied by mental illness. Responsibilities
included assisting staff in planning activities for social/recreation
experiences, conducting staff training, and evaluating staff. Also
responsible for writing Individual Service Plans (ISP) for residents living
in the home. I wrote required Quarterly Reports, Medicaid Waiver Reports,
and Census Reports for the homes to meet State and Federal guidelines.
Director of the Child Development Center of Northern Virginia: Easter Seals
Society
Powder Mills, Va.-Falls Church, Va. (12/01/00-6/01/01) Duties included
supervision of staff in the daily operation of a Day School and Child Care
Program for children ranging in age from birth to five years. The Center
provides social and educational experiences for children who are
intellectually challenged. My duties included working with the children in
the Center, assisting teachers, and approving lesson plans and activities
that occurred in the classrooms. I participated in meetings with the
executive branch of the regional Easter Seals Society and supervised staff
in-services for the center. Other responsibilities involved the
implementation of policy, including staff reviews, employee shift
scheduling, interviewing prospective employees, new employee
hiring/orientation, and financial record keeping/documentation to operate
within the fiscal budget.
