1.5 Maintaining records and reports as well as keeps census up to date and supplying on a weekly basis to the Merton or Ward in Charge.

1.6 Maintain billing of the ward when patients discharge. 2. The staff nurse makes sure that all patients receive a high standard of nursing care. This will be done by:

2.1 Close observation of the patient’s general condition, any deviation, deterioration or improvement taking vitals signs i.e. blood pressure, pulse rate, respiration rate, temperature and inform to the doctor if abnormal found routine care of the patient according to the need. 2.2 Proficient of recording and reporting of the patient to the concern immediately if any abnormality. 2.3 Effective care of critically ill, long-term bed ridden patient’s such as turning from the side to side, care of bedsore, measurement of the patient’s intake and output and recording it well. 2.4 Initiating and providing nursing assessment, planning care of inpatients. Main activities and responsibilities 2.5 Attending ward rounds with the Medical Officer/ Hospital in Charge and advising on and giving nursing care to improve the patients wellbeing. Keep good relationship with all medical students, during their clinical time in medical ward and provide necessary information to all categories according to the need. 2.6 On discharge, be responsible that all patients receive their discharge medicines and understand how to take the medicine and follow-up date.

2.7 Recording accurately medicines dispensed to patients on discharge, receiving and recording accurately payment for dispensed medicines.

2.8 Check the consciousness of the patient and management of the pain well. Observation of the general condition & carry out the doctor’s order. Keep the patient in proper position according to the need of the patient. Insertion of the Cannula, urinary catheter, giving different kind of enema for the bowel preparation, Ryle’s tube insertion etc.

2.9 Capable and perfect of providing any kinds of procedures such as I/V, I/M, S/C administration of order Drugs by Doctor, Ryle’s tube feeding, Nebulization, Oxygenation and suctioning. 2.10 The candidate should provide emergency medical care to patients as per the instructions of physician or surgeon.

2.11 Provides primary cares to inpatients assigned to him/her including performing a history and physical & assist to Doctor for dressing and maintain daily progress report of patient after post surgery and help patient for Physiotherapy.

2.12 Send & collect all investigation order by Doctor on ward round. Provide health education of hygiene, nutrition and self-care to the patients.

2.13 Provide special care for different condition such as patient with traction, dressing, immobilization, active movement and physiotherapy etc.

2.14 Frequently observe swelling, tenderness, drainage, bleeding, pulsation & elevated part. Provide knowledge about prosthesis, walker and crutches to re-establish. 2.15 Responsible to care of all types of mental illness patient as well as provide care for different type of psychiatric condition. Such as Schizophrenia, Depression, Personality Disorder, Drug dependency / Alcoholism, Psychosis manic Depressive Psychosis, Hysteria, Psychosomatic, Anxiety, Mental retardation etc.

2.16 The person should ensure that the physiological, psychosocial, safety, learning, rehabilitative and spiritual needs of the patient and his/her family are addressed. 2.17 Care for patients with psychiatric disorders, medical issues, or substance abuse problems in hospitals and assess patients, create treatment plans, and manage the patients’ care. 2.18 Administer and monitor any treatment regimens as specified by the psychiatrist, such as certain medications or activities.

2.19 Giving Psychological supports to the patient especially for severe Depressive patient and Reassure the patient according to their problems. Observe the patient behaviour constantly and close observation about disease progress, drugs side effects. Administer all prescribed medicine at right time right route, rite dose, right medicine and right patients. Maintain nutritional requirements 2.20 Provide cares according their need such as physical need psychological need, recreational need, occupational need, family therapy etc. Maintain good IPR with patients, help to re-establish or regain his interest and reduce the disability and handicaps effect. 2.21 All sharp objects, glass or mirrors items, belts, ties, matchboxes, knife, have been remove from patients environments.

2.22 Responsible to provide care before, during, and after ECT. Prepare patient and all instruments before giving ECT like bed side screen, well-padded beds, railing, small pillows to put under the neck, mouth gag to prevent injury, tongue spatula, ET tube, sterile suction, catheter, a tray with ECT machine jell, spirit swabs, normal saline, electrical plugs, sterile syringe, resuscitation tray, BP set, oxygen cylinder, toilet facility etc.

2.23 Provide psychotherapy include cognitive therapy, group therapy, family therapy, behavioural therapy, individual and psychodrama therapy. Maintaining personal hygiene, supplements diet with vitamins and minerals

2.24 Communication skills in dealing with women who have suffered miscarriages or intrauterine deaths. Evaluation of a pregnant and non- pregnant women as well as provide emotional & psychological support.

2.25 Care & management of all type of gynaecological problems and diseases as well as orthopaedic disease and problem.

2.26 Pre-operation and post-operation patient care, changing surgical dressing, assisting doctors to remove suture and drain tubes, changing position time to time to prevent bedsore as well as to provide comfort to the patient.

2.27 Doing different kind of procedure like taking ECG, monitoring vital sign, and recording & reporting of the patient complaints and abnormalities immediately to the related surgeon or physician. 2.28 Assisting women having hysterectomies, as well as those who are being treated for cancers that affect a woman’s reproductive system also provide care and help for women who have lost their babies through miscarriage or stillbirth, as well as assisting women throughout pregnancy and through the birthing process.

2.29 Provide knowledge about family planning, preventive measures of sexually transmitted disease, personal hygiene, breast self-examinations, etc. Maintain fluid electrolytes and nutritional need of the patients. Give perineal care daily by clean water and paint by Betadine. Observe drainage if present and maintain total drainage amount. Watch of any complication. Deep breathing & coughing exercise

& ambulation from post – operative 1st day.
