DUTIES AS A DIRECT CARE GIVER

Following nursing process in assessing, planning, implementation and evaluating daily individual patient care.

Maintaining patent airway performing tracheal, Oral and nasal suctioning as required. Adequate chest physiotherapy to mobilize the secretions. Two hourly changes of positions, giving steam-inhalations, mobilization, administering Oxygen.

Assessing physical, psychological needs of the client. Collection of the personal, social, medical and general data by interviewing formally and informally during various nurses client interactions, review of records, diagnostic tests. etc.

Monitoring vital signs of the client.

Meeting hygienic needs of the patient (mouth care, hair bath, and sponge bath)

Dressing (e.g. bedsore, surgical wound)

Maintaining records and reports

Provide health information to the client & the family and participating family in the care of patients.

Teaching using various methods

Post operative care-specific, post operative care according to the surgery done

Care of patients on ventilator

Maintaining patent airway

Set the ventilator parameter according to the patients respiratory status and constant monitoring of the patient

Emergency patient care

Resuscitating during cardiac and respiratory arrest.

Assisting and performing CPR.

Assisting Endo-tracheal intubations.

Monitoring Fluid & electrolyte balance, interavenous canulizations, administering intravenous fluids, monitoring and maintaining adequate record of intake and output, sending blood samples for investigations and evaluating the lab values.

Analyzing arterial blood gas sample for PO2,PCO2, Na, K, Ca, saturation of O2, Hb, SBC, HCO3, base excess, blood urea, pH etc.

Intra arterial pressure monitoring through the transducer lines.

Pressure monitoring using NIBP connected to bed side monitors or transport monitors.

Monitoring blood glucose values-beside blood glucose & urine sugar, ketone are monitored with Dextrostic and with use of Glucometer.

Hourly maintaining a bypass chart of chest tube drainage ionotrope flow rate and concentration urine output, IV fluids, heart rate respiratory rate respiratory rate right arterial pressure, left arterial pressure, arterial blood pressure, central venous pressure, peripheral temperature, oral, sublingual, intravenous, intra dermal, subcutaneous, intra muscular, intra tracheal, intra cardiac, administrating drugs and fluids via infusion pump.

Care of dangerous drugs and their records.

Doing admission, discharge and transfer of patient.

Monitoring Glasgow coma scale – The level of consciouseness is assessed by using Glasgow coma scale. Pupillary response to light also monitored changes are hourly recorded and reported.
