WORK EXPERIENCE:
The transition of pharmacy from a traditional to a clinical- focused and service-oriented profession has proved not only challenging but exciting. Performing interactive Patient Care Initiatives aimed at assisting patients to become more pro-active with their health and prescription drug management has proved mutually beneficial. In light of this, I have provided Medication Therapy Management in community settings.

Manage the efficient review, dispensing, packaging and delivery of orders to nursing homes
Prepare and check infusions for onward delivery to nursing facilities
Ensure that medication orders are appropriate and the right dosages and quantities
Check for potential adverse drug effects and interactions and effectively communicate with the nursing homes clinicians and medical group
Provide emergency medications to provide prompt therapeutic outcomes as well as ensuring continuum of care
Work collaboratively with nursing home and medical staff to optimize residents health outcomes
