Responsible for acute care and follow up care for physician practice while staff are on holiday and sick leave. Care Improvement Plus-Baltimore, D/B/A Linde Health Care 06/15/2010 to 03/15/2011

Nurse practitioner, locum tenens, hired full time to travel the Eastern portion of Georgia to perform a 12 -15 page in home physical exams on chronically ill enrolled recipients from Medicare Advantage to Care Improvement Plus. Traveling up to 700 miles a week for Care Improvement Plus who now insures e members exclusively with congestive heart failure, end stage renal disease, and diabetes. The majority of CIP members lived in either remote and /or public housing areas. Assessments of members also included, in home physical exams after various surgeries. Responsibilities include travel to rural areas to update medical history, medications, check vital signs, measuring wounds, assessing mental status, document blood glucose, creatinine clearance, and calculating GFR as well as making necessary referrals to support services like pharmacy, social services, ophthalmology, wound care, adult services, and emergency care if necessary. The twelve page assessment was UPS to Baltimore and CIP clients were left with a letter to take to their next primary care visit with recommendations. Additionally, social services, palliative care, diabetic shoes, ophthalmology visits, dental cleaning and review of all medications were scheduled for the client.
