•Clinical duties:

•Verifying physician orders: working with pharmacists during various shifts. Learning how to verify physician orders, checking the indications, patient labs, allergies, history and avoiding order duplications.

•Chemo Orders: adjusting the chemo order specifically for each patient according the physician order, BSA of the patient and the treatment plane. Also managing the proper storage and dilution for the drug to be perfectly effective for the patients.

•Renal Dose Adjustment: taking the appropriate decision about changing the renal cleared medications according to the hospital protocol.

•Warfarin Monitoring: monitoring the INR ratio for patients on Warfarin and making any necessary changes.

•ADR Report: Adverse Drug Reaction report is a report to monitor patients taking any antidotes and the reason why they are taking them.

•Collaborates with other health care professionals as needed for problem resolution or achievement of identified patient specific goals.

•Technical duties:

•Unit Dose: receiving the med requests, filling the labels and writing down the expiration dates. Answering the phone and helping the nurses.

•Pyxis duties: doing all the pyxis duties EX, filling the pyxis and the stuck out, returning meds without removal and all other pyxis duties.
