Provide overall nursing care to 106 residents within this supportive housing facility and residential treatment center, servicing a diverse population of patients in need of services related to mental illness, drug and alcohol addiction, AIDS/HIV, etc.

Supervise 10 case manager assistants, provide detailed training, delegate work responsibilities and ensure timely execution of tasks

Provide oral evaluations and make recommendations based on job performance

Conduct intake screenings, assess individual needs on the basis of medical records, interview, observation and examination of patients, and coordinate the admissions process

Obtain and record vital signs and screen patients for a variety of health conditions including high blood pressure, diabetes, asthma, STDs, tuberculosis, etc.

Provide detailed instruction on various illnesses and medically related challenges including medication administration and side effects, opportunistic infections, wound dressing changes, etc.

Administer and monitor medication administration as per psychiatrists' orders and reassess patients in view of changes in general condition

Transcribe physicians' orders and interpret and document laboratory reports

Execute screening directives for special programs and assist patients with acquisition of entitlements related to medical, functional, emotional, psychological, environmental, housing and financial challenges

Worked closely with interdisciplinary team members, psychiatrists, case managers, mental health counselors and program directors

Network with various social service agencies contract services related to substance abuse, anger management, domestic violence, homelessness, etc.

Acted as patient advocate and interface with medical professionals and family members on patients' behalf

Respond to life threatening emergencies and assist with administering life saving techniques
