ACHIEVEMENTS:

At AHS increased occupancy and revenues while putting in labor and expense controls to

better control overall expenses as related to other regions. OT in my region was also the best for any

region in AHS. Had the only region that did not have an IJ or SQC during my tenure. My region

also enjoyed the lowest associate turnover rates in the company.

Contracted as a 1099 consultant at Longhorn Village and developed the area’s only acute care/rehab non-hospital based outpatient therapy programs for Part B patients. Through hands on leadership and utilizing my network of area hospital CEOs doubled the center’s census in less than 4 months. Instrumental in the campus refinance through the issuance of $112M in new bonds on 10-29-12.

Was recruited to Horizon Bay in order to place together the company’s “Healthcare Products” in TX that included medical model assisted living and memory care units, Continuing Care Retirement Communities, new skilled rehab centers under development, the company’s only Luxury-high end Independent Living community with a Home Health Agency. Through this re-alignment increased occupancy by 24 pts. Opened one new skilled and one new assisted living property during this time. Gave my notice when informed that Horizon Bay was being required by a former employer (BSL).

Temporary job with Cantex and originally brought on-board to open one brand new rehab center which was very financially successful once opened. This success was due to my honing and implementation of the “Third-Coast Medicare Go-Model” where the patient profile reflects Ultra-High Rehab needs of at least 80% and a discharge destination to home of at least 90%.

At Brookdale Senior Living was on-boarded to take over multiple distressed portfolios post re-organization. With a hands on leadership style helped guide Administrators and EDs through this process and minimized leadership turnover at the community level. Also took over multiple distressed portfolios afterwards with property re-alignments based on the need to increase occupancy and profitability in the States of Texas, Oklahoma, and Louisiana. In four of five years my portfolios led the entire corporation in year over year occupancy improvement/growth.

Started Trinity Healthcare Management as a personal healthcare entity to develop a brand new SNF and MC in East Texas. In this role as President/CEO/and Owner was directly responsible for the formulation and dissemination of all policies & procedures and for entire daily operations. After successful completion of the opening and operation of the property, developed a dedicated therapy corporation (D/B/A Pineywoods Rehab) for service to Trinity Healthcare first, then later to various small area providers. Successfully sold both organizations for a substantial return on investment.

At TLC grew an existing region into a multi-state division. In two years took 24 skilled rehab centers and added a total of 27 additional SNF/AL/CCRCs through a combo of acquisitions and divestitures. During this growth mode successfully recruited, developed, and lead both States’ regional teams.

Joined UNISON as a known trouble-shooter who then led 16 properties in TX and KS through a pre-planned Chapter 11 re-org. A hands-on style of leadership further ensured key mgr. retention at the community level and during the same period occupancy was increased 20% and Net Operating Income increased by more than 35%. Also gained popularity for following the then governor in her soap-box tour of properties that intended to help her re-election campaign as a healthcare reformer.

● Responsible for New Beginnings Care SNFs and ALF in Oklahoma, Arkansas, Tennessee and Ohio.

● Responsible for the operation of 10-18 SNFs depending on the individual property’s census needs. Substantially

increased census in my first region and then did so in my next 2 alignments of properties. Also involved in new

acquisitions and building new SNF and renovations.

● 1099 contract work to prepare for survey, stabilize operations, and to increase revenues. For revenue enhancement

occupancy was increased by more than 100% in 4 months. Developed numerous additional revenue generating

programs such as outpatient therapy, in-house labs, new home health programs. Worked on the construction of a new

therapy gym, and doubling capacity for both the MC and AL units. Leadership of 220+ FTEs including 15 directors.

2010 HORIZON BAY RETIREMENT (15 CCRC/AL/IL/SNF PROPERTIES)

2011 Austin, TX Vice President of Operations

● Recruited to take over newly developed portfolio which included a Home Health Agency, 5 medical/5 memory care

model ALs, 1 luxury IL, 2 CCRCs, and 2 new SNF developments. Successfully recruited or re-organized regional

teams, stabilized existing operations with new leadership teams in multiple properties, and increased NOI from 10%

to 40%. Led on a daily basis a combo of 17 Administrators/EDs with a total of more than 2K associates.

2009 CANTEX SENIOR COMMUNITIES

2010 Allen, TX Executive Director (temp job to open a new SNF)

• Developed a “go model-Medicare/Private” SNF/Rehabilitation Center that was months behind schedule. Achieved

Fed certification within a month of employment, and licensure by 2nd month. Made a positive EBITDAR by the 2nd

month of full operation (a Cantex 1st). Produced a positive Net Income by the 6 month of operation (a CANTEX

1st). Hired and trained all staff, implemented all policies/procedures/systems. Hired a new asst, admin. and

and trained her to take over my role after deciding to move my family from N.TX to Austin.

2004 BROOKDALE SENIOR LIVING (15-24 AL/IL/CCRC PROPERTIES)

2009 Milwaukee, WI Regional Director-Operations OK, TX and LA

• Took over operations in OK prior to the merger with BSL .Exceeded budgeted NOI by over 1M in 05. In 12

months, grew occupancy from 78% to 96% (Lead the company in 2006). Overall, OM increased from 19% to 46%

(Led the Central Div and Corporation). Operationally, had multiple deficiency free surveys. In May of 06, was

requested to transfer into TX and LA operations. Had existing and new acquisition properties in both States.

Increased occupancy from 57% to 95% during that time (Lead the entire company in YTD Net Gains in 05, 06, 07,

and 08). OM increased from 9.9% to 45.5%, with TX being profitable for the 1st time. 07 NOI exceeded budget by

$827K+ (Lead company in NOI- 07, 08). Daily reports with 15-27 Admins/EDs; total FTEs often exceeding 3K.

2000 TRINITY HEALTHCARE/PINEYWOODS REHABILITATION

• Responsible for the opening of a new SNF/MC in East Texas. Developed and implemented all P & Ps.

Responsible for the management of all daily operations including but not limited to: A/R, A/P, marketing, finance,

purchasing, asset management, information services, human resources, regulatory compliance, and budgeting.

During this same period of time, developed a therapy company in the form of an LLC to provide in-house PT, OT,

ST and respiratory services. Later utilized Pineywoods to provide contracted therapy to local area SNFs.

• Recruited for a position in a growth company. Hired a Regional Manager for my 24 OK properties, and was

directly over up to 29 in Texas. Responsible for the conversion of new properties (27), and the divestiture of (5)

poor performers. Was able to stabilize this division and ensure profitability prior to leaving to form my own

company On a daily basis had direct reports of 2 State Regional teams, and up to 29 Administrators/Executive

Directors with a total number of FTEs under my leadership well in excess of 5K within the Division.

• Recruited as a known trouble-shooter from my previous employer. Lead 16 properties in TX and KS through a

Chapter 11 re-organization. During this time, retained all key management staff while increasing occupancy 20%,

increasing NOI by more than 35%, while maintaining the best care possible (no adverse state actions-fines). After

successfully taking this Region through the re-organization, was recruited by a startup company to grow their

business in Texas while stabilizing their existing operations in Oklahoma.
