Title: Nurse Care Manager
HEBREW HOME ElderServe Health MLTC 7/12-Present
Responsibilities include Medicaid manage care for the Manage Long Term Care plan (MLTC), supervision of the community health nurses; intensive case management of for the medically complex cases and the difficult to serve patients. Scheduling nurses for case conference and case review, reporting and/or supervising case and patient details.
