MDS Insurance is an early out program for over 50 different clients. I lead a department that investigated the third party payer for the patients. I had to find out in some cases why the payer did not pay, including billing issues. In the Insurance department we had to request the payer to be rebilled, an adjustment for copay, coinsurance, or deductible to be applied, or the facility could be asked to review the charges for coding or other issues.
