Juniper House is a Nursing Home part of the Barchester Group. Which comprises of 60 private en-suite rooms finished with extremely high standard which consist of Four Units. Two Units – Memory Lane (Dementia) and Two Units which facilitate Respite, Residential, Nursing, Palliative/End of life Nursing Care.

Job roll includes; planning, organizing and managing private rooms and all resources therein. With weekly meetings with Department Head’s in order to provide Resident Centered Care. I am currently working with the Deputy Manager on a pilot scheme which has been implemented. Three of our Senior Health Care Assistants have completed a Care Practitioner Course which includes obtaining skills of venipuncture, wound care, drug administration and documentation relating to delivery of resident center care; while under close guidance, assessment/exam and essay based. With myself as one of two acting mentors which has proven successful. Also when promoted to Clinical Lead (Medical), Juniper House was a new build. Units were set up by my team under my guidance.

Key Responsibilities

Ensure a high quality nursing care is delivered; Oversee and modify patient treatment plans as indicated by patient responses and conditions;

Work as part of a multidisciplinary team with regular meetings in order to provide holistic patient centred care which extends to their families and/or carers;

Identify, assess and manage risks to residents, staff and visitors, also to ensure compliance with Health and Safety legislation and rules;

Evaluate resident’s physical, psychological and social requirements;

Identify patient care requirements by establishing personal rapport with potential and current patients;

Assure quality of care by adhering to therapeutic standards; measuring health outcomes against patient care gaols and standards; making or recommending necessary adjustments;

Dealing with all complaints received which may be made by residents, relatives or professionals;

Interviewing staff, completing appraisals and supervisions;

Ensure relevant paperwork is current and up to date which applies to nursing allowance from the local Clinical Commissioning Groups (CCG) and Primary Care Trust (PCT);

Assess, discuss care requirements with funding bodies while adhering to Care Quality Commission (CQC) Standards;

Complete and update DICE assessments for each resident, in order to determine staffing also assess staffing is safe and adequate as residents requirements change;

Ensure the order process of monthly medication is accurate by liaising with General Practitioner, Pharmacists and Technicians in order to ensure process is run smoothly and accurately;

Order stationary/medical supplies/equipment ensure they are serviced accordingly, while keeping within budget provided;

Assessments of new potential residents at home/hospital/hospice. Completing a full assessment in order to determine which part of the home is suitable and to ensure current care needs may be met;

Arrange and attend regular Reviews with residents and relatives;

Supporting and educating residents and relatives through Palliative Care/End of Life when appropriate and communicating with the relevant Health Care Professionals ensuring that appropriate care and medication is put in place;

Oversee that Drug administration is completed in a timely accurate manor complying with the Nursing & Midwifery Council (NMC) code of conduct at all times;

Being on call, in order to provide support for Nursing Staff on shift;

While providing leadership and support to staff, acting as a mentor for new Nursing Staff. While arranging and leading training for Nurses and Health Care Assistants;

Completing regular night checks and audits;

Identify, develop and implement quality initiatives, monitor and audit all aspects of the delivery of care;

Leasing both with Deputy Manager and General Manager;
