Summary Statement: I started my nursing career at Makati Medical Center where in I worked on full time job, 40 hours a week. It is a tertiary hospital with 500 bed capacity. I stayed in a 35 bed capacity Medical-Surgical floor as a Staff Nurse from October 2001 up to December 2003. January 2004, I was transferred in Cardio Vascular-Surgical ICU (CV-SICU/Telemetry Unit). CV-SICU is a 12 bed capacity (6 Telemetry Rooms, 6 CV-SICU beds) unit with a nurse-patient ratio of 1:1, to a maximum of 1:2. This unit caters service to cardiac cases like Myocardial Infarction, Heart failure, patient with cardiac dysrhythmias, patient for invasive haemodynamic monitoring, patient for Cardioversion, pre and post Permanent Pace Maker and Automated Implanted Cardiac Defibrillator insertion, patient with Intra Iortic Balloon Pump (IABP) catheter connected to IABP machine, immediate post open heart patient such as Coronary Artery By Pass Graft and post Heart Valve repair, immediate post Coronary Angiogram and Coronary Angioplasty patient, and patient who had Streptokinase. I was promoted as a charge Nurse (Nurse Manager II) in November 2005. I was also a resource speaker of the mandatory training for nurses entitled “Medication Administration: Drug Dosages and Solution”. This includes tablet, intravenous, intramuscular and subcutaneous drug computation and regulation. I was promoted as Acting-Unit Manager (Acting Head Nurse) of CV-SICU/Telemetry Unit by December 15, 2007. By April 2009, I was promoted as Head Nurse. I also participated in the Departmental Training of the Critical Care Progressive Care Department as a resource person for Departmental Training for the following subjects: Basic ECG Interpretation; Haemodynamic Monitoring; Care of patient for Defibrillation, Cardioversion, and Pre and Post Pacemaker insertion. April 2010, I was transferred to Total Quality Services Division as Quality Service Officer, under Clinical Performance Improvement Unit. As quality service officer, we collect data for the clinical and managerial indicators of our hospital. This was also in preparation for the first international accreditation of our institution. June 2011, Makati Medical Center was accredited by the Joint Commission International. September 2011, I was transferred to Compliance and Accreditation Unit, also under Total Quality Services Division, as Compliance Officer. We monitor hospital’s compliance to local and international accrediting bodies. June 2012, I was transferred to Infection Prevention and Control Unit, also under the same division, as Senior Infection Control Officer. As an Infection Prevention and Control Unit (IPCU) Officer we are the resource speaker for “Basic Infection Control Seminar,” we monitor our hospitals compliance to standard infection control practices and perform clinical audit to find out the root cause analysis of non-compliance to standard and monitor their improvement plan in relation to root cause analysis. We do surveillance monitoring of infection rate hospital wide, especially in critical care areas (Intensive Care Unit). Reporting of reportable communicable diseases to local department is also part of our daily monitoring as an IPCU Officer. In August 2014 our institution, Makati Medical Center, applied for Joint Commission International reaccreditation which gave us a positive outcome (waiting for official result).

1.Staff Nurse: Medical/Surgical Floor-

Maintains attentiveness to customer’s needs; initiates steps that will comply with their requirements to meet and exceed their expectation.

Obtains comprehensive data from the patient’s health history and physical examination utilizing standards of assessment, use monitoring equipment and diagnostic results.

Utilizes focused assessment skills during health history taking and physical examination of the elderly patient. Executes plan of care utilizing nursing standards and geriatric care protocols.

Performs safe nursing practice in the care of the elderly :

a. Anticipates for any need/equipment needed to care for the elderly patient

b.Identifies sudden changes in the elderly patient’s health condition, implements appropriate interventions and if needed, report to proper authority promptly. Modifies plan of care as indicated by elderly patient’s response and condition.

c.Safely transfers the elderly patient.

Demonstrates effective infection control practices conscientiously (ex.: hand hygiene, waste disposal, use of personal protective equipment).

Obtains comprehensive, relevant data needed for identifying nursing problems and patient care through:

a. Interview (health history)

b. Correct obtainment of vital signs, including pain

c. Appropriate use of advanced and focused assessment technique.

d. Correct interpretation of data obtained from monitoring equipment, and

e. Correct interpretation of diagnostic results

Determines priority of nursing activities based on immediate patient’s needs obtained through intensive monitoring, focused assessment and use of advanced monitoring equipment such as but not limited to hemodynamic monitors ( Arterial line and Swan Ganz), thermodilution catheter

Implement standards of care for critically ill patient who are at risk for or with altered nutrition status, fall, pressure sore, aspiration and deep vein thrombosis.

Develops, executes and evaluates established plan of care with pain and those with specific diseases/ patient such as but not limited to end of life care, care of the vulnerable and immunocompromised patient, care of patients with communicable diseases, care of the comatose patient, care of a patient on dialysis, care of patients with ostomies, pressure sore and wound.

Ensure that:

a.A comprehensive history and physical examination is completed for each patient.

c.Results of diagnostic test and procedures are made available. Critical test results are relayed to physician within specified timeframe.

d.Patient education is conducted by staff nurses as it may be required during patient care (such as but not limited to medication administration, procedure/surgery preparation, home instruction, use of equipment, etc.)

Implements timely and appropriate corrective actions in managing safety related incidents during the shift. Escalates issues/concerns that are beyond his/her capacity to the head nurse of shift manager.

Performs random chart audit during the shift to assure complete and accurate documentation of care plan .

Actively participates in the clinical experience of the Makati Medical Center College of Nursing students. Conducts rounds or audit to ensure compliance to the Policy of Affiliation.

Supports the Headnurse in motivating the staff to embrace new projects and initiatives championed by the nursing leadership or other divisions.

Participates in the implementation of strategies t to institutionalize operational plan for the department based on the strategic plan of the Nursing Services Division.

Monitors unit standing based on key performance indicators of the division. Recommends plan of action to exceed or meet performance targets.

Role model the highest level of professionalism, compliance to policies and protocol.

Demonstrates effective infection control practices conscientiously.

Identifies stakeholders in the hospital and understands expectations.

Establishes close coordination with different unit heads/ managers in the conduction of activities related to customer service, document & data management, information management, supplies and equipment related issues/ concern and resource allocation for patient care.

Disseminates policies, regulations, circulars and programs among nurses and nursing support staff during the shift.

Identifies wastage in various processes and integrates quality tools to solve/ minimize wastes.

Participates in budget planning and preparation.

Accounts for all machine/ equipment’s functionality, completeness and maintenance every shift.

Ensures that the service attitude and standards are practiced consistently by the staff

Initiates immediate and appropriate service recovery measures to address complaints and improve patient / customer satisfaction.

Conducts a thorough investigation on patient and/or companion’s complaint. Ensures prompt resolution of escalated patient’s, companion and Physician’s complaints.

Conducts regular and comprehensive patient rounds to audit patient care rendered by the nursing staff, assess patient satisfaction, anticipate needs and minimize/address complaints.

VII.Coaching and Mentoring

Conducts bedside learning session through formal lectures, case method and safety huddles, with the staff to ensure safe nursing practice and compliance to hospital protocols is ascertained during the shift.
