Data Extraction Specialist and CAREWare Coordinator
General Description
The Data Extraction Specialist is a professional administrative position involving a range of support
duties and requiring public contact. This position has responsibility, under the supervision of the
Ryan White Program Director, for managing all medical record data extraction for purposes of report
generation and quality improvement activities. The position has lead responsibility for Area 3/13’s
CAREWare data collection interface. This employee works with considerable independence in
organizing his/her workload, maintaining required documentation, and generating required reports.
The position requires that the employee have reliable transportation. Annual mileage may be as
much as 12,000 miles (which are reimbursed at $0.445 per mile). Regular travel is required to
Alachua, Marion, Citrus, Sumter, and Columbia County Health Departments, and to Family Medical
and Dental Clinics in Interlachen.
Essential Knowledge, Skills and Abilities
Knowledge of health care services and documentation.
Understanding of medical records organization and terminology.
Ability to carefully research medical records for specific data and information.
Knowledge and experience with Access© and CAREWare computer database software,
including accurate data entry, queries, and report generation.
Ability to communicate effectively with health care providers and others.
Ability to manage multiple priorities and complete assignments in a timely fashion.
Ability to confidentially manage sensitive clinical data.

Responsibilities
Assumes lead responsibility for the development, training, implementation, utilization, data
extraction, reporting, and maintenance of the local CAREWare interface, in conjunction with
the Florida Bureau of HIV/AIDS
Coordinates all WellFlorida’s CAREWare data activities.
Confidentially receives, enters in the database, and stores all paper data submitted by the Ryan
White clinics.
Generates clinic lists, from the database, of charts to be reviewed, and faxes the lists to the
appropriate clinical personnel to have patient records pulled.
Arranges with clinic personnel for regular on-site chart extraction.
Maintains a good working relationship with appropriate clinic personnel.
Travels to all the Ryan White clinics to extract medical record data.
Enters data accurately in the Ryan White CAREWare database, using a laptop computer.
Protects the confidentiality and security of all data obtained at all times.
Generates monthly data reports for the Ryan White fiscal requisitions.
Generates reports as needed for the Quality Assurance Coordinator and the Program Director.
Other Responsibilities
Provides support for Ryan White Consortium meetings, as necessary.
Performs other appropriate tasks, as requested by the Program Director or the Chief Executive
