Your first day of employment will be March 03, 2014.*

*Your start date and orientation dates are contingent upon your successful completion of Fifth

T hird's employment screening process, which includes a drug test and a government mandated

background investigation.

To ensure that you are prepared for orientation and your first day on the job, please review the information

below and complete the necessary steps.

ITEMS TO COMPLETE BEFORE ORIENTATION

1. If you have not already done so, please accept your Electronic Offer.

2. Once the Electronic Offer is accepted, complete the I -9 registration form. Click here for instructions.

Please complete the below information and bring the confirmation receipt with you on your first day of

employment. New Employees that have not completed the I -9 registration form prior to their start date will be

required to complete it immediately following orientation class.

o Fill out all applicable fields on the next page (I -9 entry, Section 1) and click ‘Continue’

Be sure to enter your start date (your first day with Fifth Third) in the

Time: 8:00 a.m.

Location: Please meet in the MOC Lobby to begin training .

Your work schedule for the training will be 8 :00 a.m. - 5:00 p.m. Monday-Friday (March 04, 2014 – April 25,

2014). Once training is complete your schedule will be 9:30 a.m. – 8:00 p.m. Monday-Tuesday.

Take I-71 North to Exit #8A (Ridge Avenue). Take a left on Ridge Ave. and make an immediate right under

the bridge onto Duck Creek Road. Make a left onto Eastwood Drive. George A. Schaefer, Jr. Operations

Center is straight ahead.

Exit #8 (Ridge Ave.), make a right off of the exit. Make a right at the stop light (Kennedy). Make a lef t onto

Training Room Directions

T raining Room 1C:

Go down the hallway located on the right hand -side of the security desk. The room is straight ahead.

T raining Room 2A:

From security, go through glass doors and take the elevator on the right to the 2 nd floor. Turn right off of the

elevator and pass the ATM and Bistro, taking the hallway on the left. Enter 2 nd set of double doors on the

left. Training room is located inside the double doors on the left.

From security, go through glass doors and take the elevator on the right to the 2 floor. Turn right off of the

elevator the training room is located on the left before you get to the stairs.

ITEMS REQUIRED AT ORIENTATION:

In addition to the I-9 confirmation, you MUST bring proper identification with you to orientation.

On the first day of orientation, you need to bring proof of identification and eligibility to work in the

United States. All Documents Must Be Original, Signed and Unexpired . (Copies Will Not Be Accepted.)

OPTION 1 – Bring one document from List A.

List A

1. United States Passport or United States Passport Card

2. Permanent Resident Card or Alien Registration Receipt Card (Form I -551)

3. Foreign passport that contains I-551 stamp or temporary I-551 printed notation on a machine-

readable immigrant visa

4. Employment Authorization Card that contains a photograph (Form I-766)

5. In the case of a nonimmigrant alien authorized to work for a specific employer incident to status,

a foreign passport with Form I-94 or Form I-94A bearing the same name as the passport and

containing an endorsement of the alien's non immigrant status, as long as the period of

endorsement has not yet expired and the proposed employment is not in conflict with any

restrictions or limitation identified on the form

6. Passport from the Federated States or Micronesia (FSM) or the Republic of t he Marshall Islands

(RMI) with Form I-94 or Form I-94A indicating nonimmigrant admission under the Compact of

Free Association Between the Un ited States and the FSM or RMI.

OPTION 2 – Bring one document List B and one document from List C.

List B - Documents that Establish Identity

1. Driver's license or ID card issued by a state or outlying possession of the United States provided

it contains a photograph or information such as name, date of birth, gender, height, eye color and

address

2. ID card issued by federal, state or local government agencies or entities, provided it contains a

photograph or information such as name, date of birth, gender, height, eye color and address

3. School ID card with a photograph

4. U.S. Military card or draft record

Military dependent’s ID card

1. Social Security Account Number card (with current last name) other than the one that specifies

(laminated cards NOT accepted)

2. Certificate of Birth Abroad issued by the Department of State (Form FS -545)

3. Certification of Report of Birth issued by Department of State (Form DS -1350)

4. Original or certified copy of birth certificate issued by a State, county, municipal authority or

territory of the United States bearing an official seal

5. Native American tribal document

6. U.S. Citizen ID Card (Form I-197)

7. ID Card for use of Resident Citizen in the United States (Form I -179)

8. Employment authorization document issued by the Department of Homeland Security
