TBH is a large, private, single specialty psychiatric group headquartered in Napa, California with contracts throughout the state of California.
The La Casa MHRC is a long term care facility for conserved persons with severe and persistent mental illness who have not be manageable in the community. Conservatorship is a requirement of admission. Most patients meet APA diagnostic criteria for refractory schizophrenia
La Casa is a complex of psychiatric health care facilities consisting of a 190 bed Mental Health and Rehabilitation Center (long term sub-acute), a 16 bed PHF or acute facility, and a 24/7 Mental Health Urgent Care Center.
My clinical work is primarily on the MHRC where I provide direct patient care services on 3 wards with a case load of 106 patients. Additionally, I provide coverage, consultation, and administrative back up to the entire facility as needed.
As the attending physician I have overall responsibility for the care of my case load which includes the psychiatric evaluation and day to day clinical management. I provide individual psychotherapy, medication management, and liaison services for patients, other providers and care givers as needed. I work closely with our internist and forensic psychologist, community case management teams, families, conservators and liaisons from the Department of Mental Health.
I have established and maintained best practice standards of care including evidence based medication algorithms, (i.e. TMAP protocols), operant conditioning interventions for behavioral problems and liaison services to local medical facilities. Additionally, I work with a variety of students who are doing clinical

rotations at La Casa including nursing, social work, and medical students as well as residents. I also provide consultation, support and orientation to new medical staff and back up and vacation coverage to the corporate medical director and other members of the medical staff.
Approximately 85 % of the patients admitted to the La Casa MHRC carry a diagnosis of schizophrenia or schizoaffective disorder. Most of them meet APA criteria for refractory schizophrenia. Common comorbid conditions include substance abuse, personality disorder, PTSD and other trauma related diagnosis, other psychotic conditions resulting from encephalopathy, including HIV encephalopathy, brain trauma from head injury, developmental disorders, mental retardation, sociopathy, factitial disorders, and malingering.
Many also have chronic and severe medical problems requiring close co-management with our internist and, often liaison consultation to local and regional medical centers.
My original work assignment for TBH was as a full time contract psychiatrist at Foothill IMD in Sylmar, CA. where I participated in a specialized treatment model for chronic refractory schizophrenia as part of the Los Angeles County Long Term Care Pilot Project.
Prior to my current deployment for TBH at La Casa, I have worked at a variety of other locations for TBH clients. In 1999, my initial assignment was at the Foothill IMD in Sylmar on a project for Los Angeles County DMH which had been entitled “The Pilot Project”. The purpose was to compare 3 models of care at 3 IMD locations and to determine the most effective model. In that capacity, I provided direct clinical care to 140 patients, implemented the Texas Medication Algorithm, and developed a computerized clinical management system. Most of the patients were diagnosed with chronic, and chronic refractory schizophrenia that had not been manageable outside of a locked long term care facility.
Upon the completion of the Pilot Project in October 2001, I was assigned to the Telecare Homeless Outreach program for Area 7 in Norwalk, CA in the capacity of Team Psychiatrist. The program is an ACT model emphasizing psychosocial rehabilitation for persons with serious mental illness.
December 1999- December 2000. staff psychiatrist for COPE/UPI. Tucson, Arizona. COPE is a non-profit corporation providing behavioral health services to an SMI population in Tucson, AZ. My assignment with COPE consisted of providing medical/psychiatric services to the case management office located at 1002 North Country Club, Tucson, AZ 85716. In this capacity, I worked as a member of a multi-disciplinary team primarily in an out patient setting. COPE was also affiliated with the University of Arizona Physicians groups and participated in various national multi-center research projects as well as research based soley at the University of Arizona.

I was self employed as an independent contractor, working solely as a locum tenens physician in the areas

of adult and adolescent psychiatry and addiction medicine. Most of my work was with the most severe forms of disorders requiring hospital level care.
During this period I became licensed in 14 additional states and in the state of Queensland, Australia. I completed 52 assignments in a wide variety of practice settings and maintained an active and current knowledge of psychopharmacology and most types of somatic and behavioral therapies.

Practice settings included hospital, out patient clinic, emergency room and urgent care, forensic and consultation liaison. The assignments were in a variety of sectors including private, public, prisons, HMO and community mental health. In addition to psychiatry, I am also trained and experienced in the treatment and management of addiction disorders. Most of my experience in this area is in the acute hospital setting managing care during acute intoxication and withdrawal states.
