serve as a Social Services Assistant supporting the
Warrior Transition Unit (WTU) in an outpatient clinical
setting on a highly specialized inter-disciplinary
treatment team working with Wounded, Ill and Injured (WII)
Service members and their Families. Implement, coordinate,
monitor, and evaluate options and services to meet complex
Behavioral Health (BH) needs of the Warrior Transition
(WT). Facilitate, negotiate, procure, advocate,
communicate, and empower the BH care needs for the WT.
Keeps the supervisor and management team apprised of

encountered in the Social Worker's area (s) of
responsibilities. Works with the WTU staff, Clinical
Social Workers (CSWs) and inter-disciplinary members in a

appropriate Soldier Family Assistance Center (SFAC) staff
to implement the Comprehensive Transition Plan Scrimmage

Phase, Scrimmage Phase, Rehabilitation Phase, and
Pre-Transition Phase. Facilitate psycho-educational and/or
support groups for WTs and their Families as needed or
directed by WTU CSWs. Document all interactions,
