Assist social/service linkage workers in creating and processing active patients for services with their

medications, fax completed applications to the State Medicine Program (HIV).

Assist supervisor with state audits, help patient with the correct contact information to re-apply for state

assistance, receive approval list of patients that are active and maintain current list of new treatments or

changes in their medications.

Inform the state med patients of what steps and contact numbers they need for further assistance with the

necessary information.

Provide assistance, guidance, direction and support to uninsured and underinsured people in need of

access to healthcare.

Conduct presentations to groups and leaders of the community about program goals and free training and

secure training opportunities and conduct workshop trainings in the community in Harris and surrounding

counties about CHIP &Children’s Medicaid along with other health care topics.
