My Role as The Ultimate Work Authority (UWA) in CTI Oilfield Services was

to be responsible for company crew members performing various installation tasks in

a safely and timely manner, control of work, managing pad site hazards and SIMOPS

(Simultaneous Operations). UWA daily activities include the review, approval of Job

Risk Assessments (JRAs), issue of work permits prior to work commencement, overall

control of work on the site, SIMOPS management and operations liaison in response to

project (LOTO) Log Out Tag Out requests. Certified in CPR, First Aid, H2S,

HAZWOPER and AED.

CTI Oilfield Services is also a hazardous material emergency spill response

coordinating company that had their crews working on the Blackhawk area as spill

technicians, for Conoco Phillips and Deavon. Cleaning spills and power washing

anything from the tanks, separator skid, any contaminated lines and areas where spills

had contaminated. Dirt is usually removed depending on contamination and sent off as

samples to laboratories. As a Spill Technician I am experience in confined space entry

and power washing for tank cleaning inside and outside tanks, wearing correct and

proper personal protective equipment. Tybex suite, rubber boots and hyper gloves

along with a face shield and a fitted air purifier respirator if/ and when needed.

Fuel/oil spills and exposures to paraffin and over 200 different compounds in

condensate. Absorbent pads and boom installation to absorb all disposal of

contaminated ground and stored in contamination drums that when full are sealed

tight and relocated at designated area, ready to be taken to processing

decontamination plant for testing.

Tx, Oilfield & Pipeline Construction

Here in the oilfield i work as a roustabout or a hookup crew member. My

general duties are to construct and run (icp) Interconnecting pipe for Oil, Water, and

Gas to flow through. In accordance to the blue prints or the P.O.D.S. Crew was to set

valves throughout the Icp in order to open and close path flow. I as team leader/

pusher was in charge of three other coworkers, my crew members, who helped me

complete the task at hand. Assisted the Crane crew members setting down on pea

graveled pad Separator Skids, Meter Runs, Oil Tanks, Contact Towers, Heater

Theaters, High& Low pressure Scrubbers. All this is piped together by different crew

who worked simultaneously to complete task at hand, safely, efficiently, and

accurately. ICP from wellhead to meter run, fuel gas skid, separator skid, heater

theaters, gas jack compressor, contact towers, to tank area for storage. Drive company

truck, Push and Lead roustabout crews of three people. Order material needed and

required. Use shovels, picks, rakes, post hole diggers, and different types of hand tools

to complete task. Company currently switched to compressor installations all around

the Eagle Ford Shell Area. Two pushers with both their crews assigned to a

compressor. Icp the coexisting pipelines to the compressor adding valves for flow

control and gauges for flow pressure and future maintenance. I am licensed (class B) to

drive the company truck with as much tool as we need for the task and with the three

coworkers assigned to me as my crew members. I’m as well certified for the boom

forklift and aerial man lift.

02-2012 - 05-2012 Classic Industrial/Scaffold Erector Rock Springs

WY/Freeport TX

Duties were to build scaffold at refinery. Forklift operator experience for

merchandise or equipment movement. Daily duties were to build scaffolds for the

different companies working at that refinery in Rock Springs Wyoming. Any scaffold

erecting and dismantling was the main priority for our company / crew Classic.

Merchandise needed to be moved from 2nd floor to an above level. A certified forklift

operator was assigned to the crew. It was mostly a team leader or a supervisor. No

regular employees unless certified for class 6 and class 7 certification of the Boom lift

and the Sit down propane gas forklift.

In Freeport TX the daily duties were mostly the scaffold erecting and dismantling.

When all the projects were close to done the supervisor or manager wanted part of the

crew to do other jobs like. Insulation, Metal, Pipe Fitting, and helping the welders with

the angle cutting, wire netting on modules currently for insulation preparation.

2004 – 2011 Glazers Wholesale Farmers Branch. TX

I started working on the warehouse as a stocker on the day shift. My duties were to

replenish the modules with merchandise and make sure all the levels were full before

going home. Any change of the location of the merchandise was done by supervisors on

shift mostly, but sometimes I made the changes. As the supervisor was busy I helped

out as much as I possibly could.

On July of 2005 I was transferred or given a much more responsible position. I was

part of the Data Entry Team. I was in the office casually dresses every day. All inbound

and outbound data was sent to the data department for it to be keyed into the system.

Intercompany Billing was one of the most important tasks this position had to offer.

Everything going to any other branches around the United States was sent through

intercompany billing. This was merchandise from as low as one case to as much as

thirty thousand cases on a daily basis. Anything received from outside the company

also needed to be keyed into a system. I went from mainly data entry to also credits,

filing paperwork, verifying with peers orders, or outgoing merchandise corrections

before billing everything out the system and into trailers. We worked with Laptops,

SFA’s (Sale Force Automation), and Work-About units which are the same as scanners,

or Tails on, just a little bit more complex. I was in charge of switching the Memory

tape for the AS/400 Server and also The Tapes for the Lotus Notes e-mail server. All

the Glazers Companies have Lotus Notes E-mail and the AS/400 server system. I was

also working with 6400 series printers. It was my duty to clear the jam and

troubleshoot the 6400 series printer through the AS/400 menus. There were at least

eight printers I worked with on a daily basis.

On April 2010 I was transferred to The Transportation side of the company as a

router. All the drivers had routes in which they drove every day as part of their job. All

that I did in this Routing position was the computer routes using a UPS Routing

Matrix system specifically designed for routing all merchandise ordered to their

coexisting accounts.
