Clinical Transition Coordinator at Hackensack University Medical Center

Evaluate and assess patients for Home Care Services. Coordinate with Case Managers and Nurse Navigators. Collaborative initiatives with ACO’s, Bundles Projects, CMS, and Nurse Navigators to reduce rates of re-hospitalizations, length of stay, and enhance the care of patients in the community. Primary assignment areas: Emergency/Trauma Department and Observation and Holding areas. Oasis completion, Medicare and Medicaid Guidelines and Statutes. Joint Commission Survey. Implemented a s Pilot Weekend Liaison coverage at Hackensack University Medical Center – pilot evolved into a two year program and netted a large volume of home care referrals not being addressed. Re-admission rate, specific to the weekends greatly declined due to immediate access to a Home Care Liaison who coordinated the start of home care on the weekend, thus an acute hospitalization or forty eight hour hold was averted. This boosted ratings with CMS and maintained the parameters for the ACO’s in much more acceptable margins. The final outcome was the hiring of two full time Liaison’s for the weekends at Hackensack University Medical Center with the future opportunities being discussed to place weekend Liaisons and all Hackensack University Medical Center collaborative acute care facilities.
