Effectively managed and treated approximately 275 Congestive Heart Failure (CFH) patients assigned to my
team in collaboration with a physician. Responsible for assessment and treatment of CHF according to the
American Heart Association guidelines. Performed patient assessments to include routine and non-acute
appointments, as well as acute appointments requiring hospital admission. Completed comprehensive history
and physicals at admission to the clinic and with each appointment. Reviewed laboratory and diagnostic
