Effectively managed and treated approximately 275 Congestive Heart Failure (CFH) patients assigned to my

team in collaboration with a physician. Responsible for assessment and treatment of CHF according to the

American Heart Association guidelines. Performed patient assessments to include routine and non-acute

appointments, as well as acute appointments requiring hospital admission. Completed comprehensive history

and physicals at admission to the clinic and with each appointment. Reviewed laboratory and diagnostic

studies including CXR, cardiac catherizations, echocardiography studies, etc. Developed, initiated,

implemented, and modified treatment plans including titration of medications. Planned and ordered
