I am currently employed at UFP TECHNOLOGIES in Kissimmee, Florida. I've

been working for UFP since Feb. 2008. I'm the production/maintenance

manager at our plant.

.

My job description is to, but not limited to maintaining our production

line machines; to include, develop, maintain and document a preventative

maintenance program, troubleshoot and repair, order parts and materials and

keep logs.

As previously stated, I maintain manufacturing machines to include the

following:

1. CNC Multi-Cam, Multi-Head Router Machines (4 each).

2. CNC Hydraulic Presses (Freeman-Schwabe, Hudson and Bruno).

3. Maintain all glue machines (five plus Machines).

4. Automated Billet Welders (two each).

5. Jet Edge Water Jet Multi-Head CNC Machine to include it's closed loop

water filtration system.

6. Horizontal Saw (Edge-Sweet) CNC.

7. Vertical Saw (Baumer) CNC.

8. Horizontal Wire Saw (Baumer) CNC

9. Band Saws (five each).

10. JTW Compactor and conveyor system.

11. D-1 Slicer Machine (two each).

12. TTARP Press and PSA Machine.

13. Bailer, two Fork Trucks, pallet jacks (two each) and all related

support equipment.

. The packaging is produced from an array of foams, plastics and

specialty fabrics (depending on specific protection needs).

. Extensive fabrication and vacuum forming capabilities are used to

combine foam materials with a wide variety of rigid and corrugated plastics

while laminating capabilities gives the ability to adhere foam to many

different types of fabrics including ballistic nylon, circular knits,

Cordura , Sapandura , and pack cloth.

. Materials used in manufacturing include PE foam, polyurethane

foam, reticulated polyurethane foam, specialty foams, ABS, APET, Kydex ,

PETE, PETG, PP, PC, and PS.

. Maintain 37 production line machines to include:CNC multi-cam,

multi-head router machines (4 each); CNC hydraulic presses (Freeman-

Schwabe, Atom, Hudson, and Bruno); 5 glue machines; 2 automated billet

welders; Jet Edge Water Jet multi-head CNC machine to include its water

filtration system; Edge-Sweet CNC horizontal saw; B umer CNC vertical saw;

. Develop, maintain, and document a preventative maintenance

program using a computer with a manual backup to include a visual display

board as a ready reference, along with an information sheet attached to

each machine for the operator to view as a ready reference as to the

maintenance performed at any given time. This form would be called MDR-1

(Scheduled Maintenance hourly/calendar)

. Troubleshoot and repair. This type of maintenance would be

documented on a MDR-2 form (unscheduled maintenance for the purpose of

gathering data to foresee future breakdowns thereby scheduling high time

items.

. Order parts and materials. The use of MDR-2s assists in setting

up a parts stock room as we will know when we can expect a breakdown of a

particular part so as to keep these parts in my immediate inventory,

preventing long delays due to parts on order that may take weeks to arrive

(less down time).

. Keep logs up to date to ensure maintenance for scheduled and non

scheduled maintenance is properly documented.

. Also responsible for building maintenance, i.e. building

electrical, to include installation and relocation of production machines;

installation of phone and modem lines; and plumbing.
