The Caring Together program is a community based program to prevent, educate, treat, and maintain patients
with HIV and/or AIDS. On a daily basis I performed many duties, this is a multi-functioning position. I perform
walk-in rapid HIV testing, routine HIV blood draws, maintain contact with the pts to make sure that their
individual needs are being met, follow the HIV doctor during pt visits, make sure necessary paperwork is up to
date as required for program funding, schedule follow up visits, go over each pts chart prior to visit to make
sure vaccinations and immunizations are current. If a person has a reactive rapid test, then it is my responsibility
to draw the confirmatory lab work, get in touch with our HIV provider (doctor), set them up with an initial visit
with me, at which time I would gather all pertinent information, including PMH, family medical history, Proper
consent forms signed, set the person up with a case manager for financial reasons, and look at their risk factors.
If warranted, I would contact the DOH for partner notifications. Compassion and being educated are critical for
this position to help make the next steps for the pt as smooth as humanly possible. I am their support. One of the
most important aspects of this position is to educate the pt about their new diagnosis. Once diagnosed, staying
in care and being compliant with medication is key the staying healthy and leading a life as normal as possible.
