Position Social Worker. This position was a 200+ building. The charts were
totally electronic. The Administrative staff including Social Workers and Charge nurses were
provided with laptops. Social Workers was responsible for their section of MDS, care
planning; and working with probate court regarding conservatorship issues, Assessed family
dynamics to be assured resident will be discharged to a safe placement. I also worked
closely with the VA to assess if resident or spouse were entitled to benefits which did result in
some residents appropriate for VA Benefits for nursing home placement; some spouses were
entitled for supplemental income because a spouse was in a nursing home. It’s a great
program and I was happy to work with the VA rep. I also did discharge planning and set up
services and equipment for home. I was very happy with my employment there but I
decided to move to California to be near my grandchildren.
1999-2000 Homestead, in Stamford Connecticut
I was the Administrator. Responsible for the overall operations of the facility.
This building was challenging however with organization skills and commitment I was able to
resolve most issues.
1995-1999 Woodlands Health Center, Waterbury Connecticut
My position was Director of Social Services; Admissions Coordinator&
Marketing. It was a 90 bed facility which was why I held various roles. As a Social Worker I
was responsible for overall psycho social assessments; addressing complaints; family
meetings ; help new residents and families adjust to nursing home living, as well as care
planning and various other duties. As admissions I was responsible for the waiting list, tours,
signing in new admits. As the marketer I was responsible to go to hospitals and other
appropriate agencies to provide information of services offered at The Woodlands. I
resigned to pursue my Nursing Home Administrator License.
