Objective: To Obtain a full time position as a Family Nurse Practitioner . I have a large

skill set I have gathered through education and practice. The basic skill set I gained in

u ndergraduate school, assessment, planning, intervention, and evaluation, was fur ther

enhanced by my Masters Program. I am well organized, enjoy being autonomous (I will

a lways ask someone if I do not know) and work to fulfill my moral, ethical, and duties as my

Scope of Practice Guides me. I am a strong patient advocate. My philosophy is much the

same as i t has been since starting my career, to care for patients and their families with

d ignity, professionalism, education and respect them through their lifetime spectrum. As

patient needs change, I change my approach to best meet their needs and continue to be

09/14-11/2014: Bethesda Community Clinic NP ( Recertifcation Clinical Hours)

Worked with an all Nurse practitioner ran clinic for the underserved and underinsured

population of several counties. 160 supervised clinical hours to reassess skills as a Nurse

P ractitioner. After approximately two weeks, I was seeing patients on my own and only

having the other NP’s there to ask questions and for them to sign off on my prescriptions

and sign off on my charts. Utilizing limited resources and the resources available to me,

rendered t reatment to all ages across the lifespan. A large population of these patients

were diabetic, hypertensives, hypercholesterolemia and other chronic illnesses. Well

woman visits STI’s, Urine drug screens for known drug abusers in a program requiring

close monitoring and many other Acute and chronic management. Other skills included

i nterpreting and ordering appropriate labs and other diagnostic tests, adjustments of

medications based on labs.

Currently my patient load I carry is 20-30, depending on walk ins.

07/12- Current: L ife Plans: Field Assessor: (RN)

P roviding in home assessments for clients who have recently been hospitalized to ensure

a ll disciplines are in place to ensure clients needs. Providing GDCH in home assessments

for Untied Health Group ( 6 month assessments).

06/12- current : Providant Wellness Services: R N

I n home assessments, wellness clinics, educational classes on obesity, cholesterol,

D iabetes, Dexa Scans, Immunizations, Supervised 12 other clinical staff members, Flu

and wellness clinics.

12/10 – 05/12 T he Judge Group : F ield Case Manager (RN)

Primary responsibili ties including enrolling patients into a program provided by their

i nsurance company. The program provided them with access to a nurse 24/7. I did initial

and ongoing detailed physical assessments in their home as well as medication

reconciliation, reviewing medications for misuse/overuse/ and ensuring the patient was

k nowledgeable of the purpose of their meds. Other responsibilit ies included ensuring the

patient was in a safe environment, make proper referrals to appropriate modalities as

needed such as doctors, meals on wheels, Social Services, and Adult Protective services if

absolutely necessary. Provided education on fall prevention, proper lifting, and kitchen

safety concerns. Monthly phone calls were made to f/u on patients to inquire about their

particular diagnoses such as blood pressure, blood sugar last MD and future MD visits and

t he results, following labs pertinent to the patients condition. The goals of this program

were to give the patient a nurse to speak with hoping to catch early problems provide

p rimary and secondary preventivon education, and intervene to keep the patient at home,

reassure family members and the patient . Post op evaluations were performed in

collaboration with other modalities such as PT, Home Health, or Hospice.

12/08-12/10: Medical leave due to a near fatal Seizure. Unable to work and surrendered NP

l icense due to the inability to work. Began working in 2010 as an RN. 12/!# Retested and

became Certified. Currently finishing a required 160 hours of Family Practice Re entry

P rogram, which will be completed November 6.

04/05 -12/08: Community Health Clinic ( NP)

Primary Family care rendered to all age groups across the lifespan. Ordering and

i nterpreting labs.

Treating and controlling both acute and chronic illnesses. HTN, Dyslipidemia, and

D iabetes type 2 are only a few of the conditions I have managed. Writing prescriptions as

appropriately indicated for patient needs. Encouragement of lifestyle changes and strong

focus on wellness areas strongly emphasized in my plan of care. Especially in A Family

P ractice setting,, building t rust and the ability to focus on continuity of care are the

concrete stepping stones to a successful practice. My other ancillary skills are drawing

b lood, Vital signs, etc. Recognition if a patient needs care beyond my scope of practice and

referring them to the appropriate expert.

04/03- 04/05: A rista OB/GYN: NP

P rovided Well patient OB/GYN care. Duties included: Providing care to OB patient

ensuring appropriate preventive and maintenance care was provided, prescribing of

appropriate meds, labs, pelvic exams/cervical checks, ensuring appropriate f/u care was

p rovided if a problem was suspected or known during pregnancy. Other duties included

hospital rounds, patient admission and discharge orders. Performing GYN exams,

educating teens regarding pregnancy prevention and methods of contraception. Treating

appropriate illnesses such as STD’s UTI’s, various illnesses, prescribing of meds for these

conditions, maintaining follow ups via phone or appointment as necessary. Addressed

Family Practice issues with pregnant and nonpregnant clients especially thyroids,

gestational diabetes, hypertension, I &D wounds, sinusitis, streptococcal pharyngitis etc.

01/01 -04/03: E mbracing Hospice NP

Provided home nursing care for a variety of patients with many different diagnoses.

Patient load 20-25/week. Attending IDT meetings, collaboration with a multidisciplinary

team to make sure all of the patients needs are met. Responsibilit ies included medication

management, comfort care, assessment, organizing Hospice team of CNA’s, Chaplain care,

respite care etc to meet both the family and patient needs. Other responsibilit ies included

patient and family education regarding Hospice services, disease processes, and supporting

t hem emotionally during the dying /grieving process. Keeping patients charts updated,

a ttending interdisciplinary meetings, and keeping the Medical Director informed of

patients needs and well being. Adjusted patient care based on progression of disease

p rocess and family needs.

03/97 – 03/99: R ichland Memorial H ospital Labor and Delivery RN/PACU

This Labor and Delivery Unit specialized in primarily high r isk care for sick mothers and/or

u nborn babies. This included multiple gestation, preterm labor, HEL LP management, fetal

hydrops which required int rauterine fetal blood t ransfusions, preterm rupture of

membranes, int rauterine fetal surgery. My duties included both maternal and fetal

assessment, reading fetal monitor strips, management of multiple IV’s, both scrubbing and

circulating during cesarean sections and other surgeries. ADON responsibilities for one

year included staffing, budgeting, developing and implementing new policies and

p rocedures, holding continuing education classes. Taught incoming nurses and students on

how to interpret fetal heart rate strips and UC monitoring with both internal and external

monitors. Responsible for both scheduled and state cesarean sections as well as recovering

t hose patients. Pre eclampsia and eclampsia required management of Mag sulfate and

P itocin together at times. Maintained NRP certification as well as AWHONN certification

i n preceptorship in L &D. Due to the severity of the illnesses of some of the unborn infants,

babies were attended to in both N ICU and well baby nursery. This was not an LDRP

situation, although if the L &D nurses were not busy we f loated through all units on Mother

Baby. When a well baby was in the room with mom we were expected to care for the infant

i ncluding vital signs, recording feedings and assess cardiac and respiratory activity.

E ducation of the mothers was done as much as possible before beginning any procedure.

T his RN taught 2 prenatal classes each 6 weeks long) every other week. Assisted as a

p receptor for new L &D RN’s as well as having Senior Nursing Students follow me

t hroughout the day. The hospital had an active four bed OR for scheduled and emergency

cesarean sections. The OR included a 5 bed PACU unit. Int rauterine cord t ransfusions

were given for Fetal Hydrops . This RN acted as the Bereavement coordinator as well.

A fter a fetal demise, light baths were given, small clothes and diapers were placed on the

i nfant and i t was hand and footprinted. The mother was allowed to hold the infant as much

as she wanted to after she had been taught what to expect.

03/91 – 03/97 N orth Fulton H ospital. Med/ Surgical Unit RN.

( First position as an RN) Primary duties included post op management, medication

management, ensuring excellent patient care and developing new skills. I also managed

I V’s both central and peripheral lines.
