My name is Mary Ann Diwas, I am applying for the part time position as a Personal

Support Worker.

I am presently a PSW student taking the PSW program at St Louis Continuing Adult

Education Center. I completed six weeks (195 hours) of co-op at Wellington terrace Long Term

Care in Fergus. I will be finishing my community placement at Guelph Living Independent in

Guelph, (125 hours) on November 10, 2014. My PSW course at St Louis Continuing Adult

Education Center Cesba exam is on November 14, 2014 and Graduation is on November 21,

2014 that is when I will receive my certification.

Skills I have developed within my co-op are transfers and lifts, assistance with feeding,

tub bathing, showers, assisting in ADL’s, providing personal care and building good

interpersonal relationship and rapport with the residents and staff.

I feel I am very professional, dependable, I am not difficult to work with, I recognize and

provide individual care to residents, I treat my residents with dignity and respect and I always

promote and incorporate the DIPPS (Dignity, Independence, Preference, Privacy and safety)

Principle in my daily care with the residents.

My goal is to make the residents happy every day, provide comfortable care and provide

safety care.

I will be a benefit to your organization because I am motivated, hard working person, I

am gentle, caring and loving person, and would be a true team player. I have a huge heart and I
