This project is the Insulation of two 275,000 barrel oil holding tanks with Dura Joints one piece insulation

product. The project required the guidance of a senior safety advisor to build the contractors project

safety management plan, and the corporate safety manual as well as completing the ITP for the c ompany

to ensure quality control. Leading, mentoring the company’s and field leadership to develop an active

positive safety culture to ensure a safe project.

Installation of 65Km 16in pipeline Duties included but were not limited to writing the of the site specific

and environmental orientations as well as issuing safe work and ground disturbance permits to employees

and contractors as well as new hire orientations to all workers coming on site. Touring the work areas to

ensure company safe work procedures were followed during work execution. Liaison with client safety

and environmental representatives to ensure project requirements were followed. Completing formal and

informal inspections and of equipment and the work area as well the safety administration on a daily

basis for compliance to the clients expectations. Resolving short falls observed during field inspections.

Completing equipment administration inspections to a high standard ensuring documentation was in

place and correctly completed for operation.

Agrium Nitrogen Operations May 22-June 09 -2014

Project Night Safety Advisor Boiler Rebuild

This project was brought to my attention as Agrium had several First Aid events during the completion of

the project, reporting to the project engineer the duration was of a short time period approximately two

and a half weeks. During this period I was brought in to be the client (Agrium) representative, having the

contractor safety reps reporting to my office. For the period the project field tours with the contractor

safety reps were completed and safety short falls were addressed and corrected. Meetings with the

contractor safety reps were held to address issues pertaining to the project through the project twice

weekly to assist the contractor reps with the challenges at their level to quick resolution. On completion

of the project no incident or injury events where reported and the project was co mpleted with great

satisfaction of Agrium and start-up was attained with no incident’s

This project focused on the dismantling of approximately 150,000 tons of scaffold materials f rom the

construction contract.

Reporting directly to the corporate HSE manager I was being brought in to the project to replace

existing HSE leadership

Led and assisted the project management and field leadership, taking the advance in leading and

developing the safety culture as well as mentoring General Foremen and Foremen in incident

investigation and assisting them in understanding their requirements under provincial legis lation.

Up grading and placing policies on site in place as needed as well as adding safety systems to

increase functional ability of the company project HSE system.

Leading the HSE group of four employees and guiding company field staff, in the devel opment of

CSE rescue plans and requirements to complete entry work safely.

Attending client senior HSE Lead meetings with the client and other contractors.

Reporting directly to senior Shell HSE representatives on Incident status and corrective

measures.

Coordinating safety efforts for the company in field activities including but not limited to the stop

the drop program.

development of a strong employee safety culture. This contracted position was leading the night shift on

the condenser re-weld project and the furnace shut down and rebuild reporting to the night shift

superintendent and Suncor HSE leads.

During the course of the furnace rebuild

Meeting with Suncor Senior HSE leads and senior construction management as well as

coordinating HSE functions including Confined Space Entry work with other contractors to and

including field Inspections of all site areas and plant activities coordinating and leading Incident

investigations with subcontractors.

Attending Suncor HSE lead coordinators meeting to discuss current trends in Incidents, group

Incident review meetings and coordinating training requirements for field leadership and

employees.

Also directly in charge of all HSE administration functions including leading and mentoring field

leadership to accountability of field leadership in a successful project. Meeting with, Suncor

senior night shift and liaison with regards to all HSE matters including reporting to Suncor

management during the course of the project.

During these projects which entailed the cutting and inspection and re-welding of the condenser bends

for a total of 1600 units completed without incident, leading Melloy to m eet Suncor’s journey to zero

achievement.
