patients, and teaching all aspects of diabetes. Teaching CHF patients to weight themselves daily, if able

Had 1 patient to do private duty with, fed him orally. Pt had a trach that needed to be suctioned several

times a day, trach care. Bathing changing all air tubing and hanging new.

All About Family Homecare May 2012-Feburary 2014 Case Management of 35 patients a week 5-7

patients a day . Changing PICC line and Broviac Line dressings, blood draws from lines, teaching

Pinnacle Senior Home Care 3/13/2014-7/16/2014

Care Managing 30 patients, 5-6 per day, with severe dementia, congestive heart disease, COPD patients,

diabetic patients, malnutrition patients.

Divinity Home Care 7/21/2014 to present

Case Managed 7-9 patient s per week, for diabetic care, wounds care, wet to dry wound dressings, care of

chemo patients, CHF, pulmonary edema.
