As Administrator at Greenville Health and Rehab, I was responsible for all the day to day operations of
this 120 bed facility. During this time, the facility underwent an expansion to include offices and a new
therapy gym which allowed out-patient therapy. Greenville Health and Rehab, in addition to serving
Medicare/Medicaid/Insurance clients, it also contracted with the VA to provide services to veterans.

Smith Nursing and Rehab closed about a year ago. It was a 60 bed facility in a small town. It was very
quaint and very well regarded in the community. Smith provided excellent care and received excellent
survey results. Smith had a very dedicated staff, many of whom had worked there for 20+ years.

I served as Administrator at Greenville Health and Rehab, a skilled nursing facility, licensed for 120
dually certified beds. During this time the team initiated the Wii program for use in therapy as well as in
activities. As a result of the enthusiasm for the program, we were asked by Texas A&M-Commerce to
participate in a research study. The plan for the study was to prove the significance of benefits achieved
through use of the Wii program. I was able to assist the team in implementing the ‘person-centered care’
approach, i.e. menu choices, salad bar, and quality of life specialists provided for each resident.

Sulphur Springs Health and Rehab is a 128 dually certified bed facility. While administrator in this
building, the team and I were able to implement several ‘culture change’ improvements. Restaurant style
dining and the salad bar were a huge hit with the residents and staff. Residents also enjoyed the ‘spa
bathing’ experience which included aromatherapy, soft music, and massages. While in Sulphur Springs,
the team and I were able to achieve the highest Medicare percentage to date. In May I transferred to
Greenville Health and Rehab.

During the time I served as Administrator at Leonard Manor we achieved this facility’s very first
deficiency free survey. This small building is licensed for 79 beds. Many long term staff are very
dedicated to this facility and the residents who live there.
