Investigated and audited all written incident reports along with all residential restraint packets to assure all
facts are reported correctly according to the policies of the facility. Reported critical incident reports to ODMH
and local ADAMH board within 24 hours of discovery
• Initiated fact findings in regards to internal investigations. Upon completion submitted Franklin County
restraint packets into the database and sent alerts to the case workers
• Met with state and county investigators to complete external investigations
• Educated and trained staff on performance improvement needs and took appropriate preventative corrective
measures
• Performed monthly audits to assess tracking trends and patterns. Upon completion the facts are presented
to the leadership committee
• Instructed monthly new hires classes on proper incident documentation; instructed new hires CPR/First Aid
class along with refresher courses to current employees; submitted completion records to the American Red
Cross data base.
• Initiated Patient Care Assessments, admission forms, performed timely scanning/uploading patient
documents into medical records, and maintained monthly patient documentation by ensuring that all are
completed in its entirety
• Provided clerical support to nursing staff in conjunction with relieving receptionist when they were not available

Responsible for a 20 bed, all female unit working with youth from ages 12-18 by ensuring they were not only
kept safe from self but from harming peers whereas most suffered from mental illnesses
• Aided in teaching the young ladies on how to function in society along with skills they needed once they
were released from our care; Completed 15 minute checks for each of the residents on unit; Supervised the
employees that worked on the unit
• Utilized cross training skills and was responsible for a 20 bed, all male unit when there was not coverage
on that particular unit
