• Prepare discharged patient charts as required by law.

physicians meeting.
• Support and monitor Patient Care Coordinator work
1. In termediary between patient, doctors and staff requesting
medication refills, communicating lab test results as requested.
2. Request patient information from hospitals, labs specialists’ office.
• Perform Home Health Care Coordinator functions as required intermediary
between home health care staff and doctors completing plan of cares and
obtaining correct certification and recertification’s. Creating face to faces as
required by CMS.
• Assess patient vital signs, prepare for physician exam, and assist with minor
surgical procedures and/or f lexible sigmoidoscopies.
• Preform EKG’s, x-rays, I M/Sub Q injections including narcotics, antibiotics
and vaccines, TB skin tests, phlebotomies, mini-mental status testing,
• Other various clerical duties incoming and outgoing mail, FedEx, review
accounts balances for payment, multi-line phones, and recap time keeping for
A DP/payroll, order medical supplies and office supplies.
