JOB DESCRIPTION:
A. Professional Responsibilities
. Provide nursing care to injured and sick team members and
Clients
. To coordinate with the accredited Diagnostic Center in the Pre-
Employment Medical Examination, in the Annual Physical
Examination and in the health maintenance program. In the
absence of the Company Physician, to perform work activities
which are within the scope allowed by the nursing profession and
if more extensive examination are needed, to refer the same to
the Company Physician.
. Actively participate in the maintenance of Occupational Health
and Safety by attending the Monthly Health, Safety and
Environment Committee Meeting and to give suggestions in the
improvement of working environment affecting the health and well-
being of the team members and clients.
. Maintains an updated and factual reporting and records system on
the following items:
a. Daily Team Member and Guest Consultation
b. Individual Team Member Medical Record
c. Daily Medicine Consumption for both Team Members
d. Daily Accident and Injury Reports
e. Record on the regular inspection and replenishment of First
Aid Kit
f. Complete list of Certified First Aiders
g. Daily Medical Insurance Availment
h. Updated Medicine Inventory
i. Updated List of Equipment and Medical Apparatus
j. Complete file of Updated Team Members' Health Cards

. Prepares and Submits the following reports
o Semi-annual annual medical report using form DOLE/BWC/HSD/OH-

. Collects nursing assessment data and documents in a timely, complete
and factual manner.
. Administers medications to team members and vessel crew in accordance
with the Company Physician's orders. Such medication administered
must be properly documented in the Team Member's Individual Medical
Record.
. Ensures that medicines and other clinic supplies are properly stored,
received properly from the Receiving Department and accurately
accounted for.
. Prepares Purchase Requisition or Market List for medicines and other
clinic supplies in a timely manner to ensure continuity of supply.
. Maintains medical equipments and apparatus in a clean and safe manner
to ensure sanitary and good working condition. Reports any broken
equipment or apparatus or hazardous situations to the Human Resource
Manager or to the Safety Manager
. Conducts Home Visit on Sick Team Members upon instruction from the
Human Resource Manager. In the absence of the Human Resource Manager,
the Home Visit advice will come from the Division Head/Department
Manager
. Assist in the investigation of accidents or injuries to Team Members
and Clients take appropriate action to obtain medical care or maintain
the health of the affected person.
. Assist the Team Members in accomplishing their Sick Leave application
and Gate Pass once the Team Member has been advised by the Company
Physician for Home or Hospital Confinement.
. Assist the Team Members in the availment of their Medical Insurance by
liaising with the Insurance Coordinator in the issuance of Letter of
Authorization and Approved Endorsements.
. Assist Team Members in the processing of their SSS Compensation Claims
and other pertinent papers such as Philhealth, Sickness Notification
and Disability Claims.
. Closely monitor the condition of the Sick Team Members whether they
are under Home Confinement or Hospital Confinement and to give the
Human Resource Manager and respective Department Head an update on the
Team Member's condition.
. Update the Department Head of the Health Condition of their Team
Member most especially if confinement or Sick Leave has been advised
by the Physician.
. Completes and accomplish other task and projects assigned by the Human
Resource Department

A. Professional Responsibilities
. Provide emergency and palliative care and treatment to all
employees within the legal scope of nursing responsibility
in the absence of the company physician. This includes first
aid, taking of vital signs, health interview, dispensing of

. Participate actively in disaster control program, fire
prevention program, etc.
. Closely coordinate with the assigned doctor/s and other
technical personnel safety committee in the investigation of
accidents, if any, and those which are related to the standards
of occupational safety and health.
. Validate employee's reimbursement forms, sick and maternity
leave forms and other medical related forms in accordance with
the policy of the company.
. Schedule the medical and dental consultations, physical exams,
and other diagnostic procedures.
. Monitor the employees medical allowance coverage; medicines,
medical supplies of the clinic and maintain required minimum
inventory.
B. Administrative Responsibilities
. Plans, recommends or implements medical services program for
the benefits of employees.
. Creates awareness among nursing/paramedical staff on proper
budget control.
. Refers to Company Physician problem/cases for disposition and
management.
. Coordinates medical services program with other department
and/or with other community and government agencies.
. Assess occupational health problem in the company with emphasis
on the preventive aspects like recognizing existing potential
hazards.
. Plans, prepares, and monitors medical service manual.
. Maintains individual health records and keeping its
confidentiality.
. Conducts research study on specialized cases which has direct
bearing on the working conditions, health hazards and other
related conditions.
. Make suggestions o recommendations in the development
and establishment of new practical techniques, methods, systems,
and approaches regarding health and safety.
. Participates actively in the promotion of Family Planning
Program.
. Prepares monthly Family Planning Reports and submits to
Department of Labor and Employment - Family Planning Unit.
. Prepares and submit monthly reports on medical cases, accidents
analysis, hospital cases, medicine inventory report, etc., to
management.
. Facilitates referral of employee and/relatives to government,
private, and civic agencies.
