I completed my LPN training in August of 2003 and received my licence on

**/**/****. *’m very reliable as a worker and very dependable. I complete all

work that I’m to do before I leave for the day. I have worked on a TCU united at

Hilltop Center, that is a skilled unit with traces, wound vac and IV’s.

Appalachian Regional Hospital June 2,2014 to present is my current job on

Stanaford road, 3rd floor, Number 304-***-****. My job duties are to pass

medication, due assessments, Complete treatment and write order, check to make

sure all orders have been carried out.

Heartland of Rainelle July 2011 and still do prn nursing there. 606 Penn. Ave

resident and to assess them. Complete treatment and write orders if there are given.

Hilltop Center Jan. 2008 till July 2011on Saddle Shop Road in Glen Jean . My job

duties was to pass medication and do treatments. This was also a skilled unit that

requires vital signs and assessment of all resident.

I received my GED from Fayette Plateau Vo- tech on 7/20/99

Received my Medical Office Assistant training thought the mail with Harcourt

Learning Direct. Completed this with Diploma on 10/23/01

Enrolled in ACT in Beckley WV for LPN classes in August of 2002 and completed

the course in August of 2003 with passing board and receiving my LPN license.

I’m a very hard worker and willing to learn. I finish everything that is need to be

done before i leave. I feel that I would be a good addition to the staff at the VA

medical center.
