In my position as a EMT I respond on all 911 medical emergency calls in
the contracted area. I am in charge of daily operations of the EMS on the
Fire Department. This position has allowed me to attend college full time
and pursue a nursing degree while supplementing the family income.

In my position I have had to develop the safety program at 2 manufacturing
sites. I have written the company safety policies and procedures. I also
applied for and was awarded intervention grants from the Bureau of Workers
Compensation totaling $40,000 for each facility. I have directed the
implementation of the Drug Free Workplace program through the Bureau of
Workers Compensation. I also assisted in the implementation of the 10 Step
Program through the Bureau of Workers Compensation. I also conduct all the
plant wide safety training and safety audits for both facilities. I also
perform process audits using lean manufacturing as a guideline.

I have a total of 27 years experience in Quality and 12 years experience in
Safety.

In my position, I was a Quality Assurance Engineer. I oversaw the Quality
on new projects assigned to me. I put together the PFMEA, Control Plans,
Process Flow Diagrams and PPAP submissions. I make sure that all Quality
objectives are accomplished on the company side. I am the direct quality
contact to the customers. I have a strong background in the big 3
automakers GM, Chrysler, and Ford, which were my responsibility at
Tigerpoly Mfg., Inc. I also had responsibilities with Honda and Nissan.
I was also co-chairing a committee to achieve TS16949 certification.
With my back ground and experience, TPM had me conduct process audits and
which included making suggestions on correcting or leaning out the process
to make the process more effective and profitable.

At Circle Plastics I supervised 20 employees in the Quality Control
Department. I oversaw the daily activities of the department and directed
the Quality Auditors in the customer's needs in the quality spectrum. I was
the main contact for customers with quality issues. I oversaw the
Corrective Action responses for both internal and external problems.
I was also the Management Representative for QS-9000/ISO 9000 certified
quality system. I oversaw the QS9000 and ISO9000 program. I scheduled
internal audits and bi-annual Surveillance Audits through our registrar. I
managed the document control and wrote procedures and policies for the
company.
I was also the Safety, Health and Environmental Manager. I administered the
Safety Program which entails enforcing OSHA rules, oversee the Transitional
Work program, Coordinate and train all employees on required safety topics.
I also manage the governmental reporting to OSHA, EPA, DNR Department of
Water and to the Bureau of Workers Compensation. I investigate all
accidents and equipment damage. I also certify all injuries that are

. 2008 - I adjusted the painting and wiping process of painted parts, DK
Manufacturing went from a large hazardous waste generator to a small
hazardous waste generator. This eliminated the need for annual EPA
reporting. This also saved the company over $25,000 a year.
. 2008 - For the first time in both plants history DKM Lancaster plant
earned a Safety Award from the BWC and the DKM Frazeysburg plant
earned 3 Safety Awards from the BWC. This was accomplished during the
first year of the companies new safety program.
. 2008 - BWC Premium at DKM Lancaster dropped from $135,000 per year and
penalty rated to $54, 000 per year and group rated.
. 2007 - Developed the Safety Program and Procedures at DK
Manufacturing.
. 2007 - Changed how hazardous waste was being handled and disposed of
which also met EPA criteria. This was a cost savings in excess of
$30,000.
. 2007 - Made DK Manufacturing Frazeysburg a Drug Free Workplace. The
BWC awarded the company with a 10% premium discount.
. For 2003 there were only 4 record able injuries at Circle Plastics and
the
last lost time injury was February 17, 2000.
. 1997 Developed the Safety Program and Procedures at Circle
Plastics Products, Inc.
. 1997 Circle Plastics had 53 record able injuries and 4 lost
time injuries in the OSHA Log. In two years these were
reduced to 12 record able injuries and 0 lost time injuries.
. I developed the transitional work program for Circle Plastics.
. Circle Plastics went from OSHA's 10,000 dangerous
facilities list to being awarded multiple Safety Awards
the past four years.
. I assisted in the reduction of the BWC Premium at Circle Plastics by
