program (DSME) with an emphasis on appropriate health literacy utilizing the
National Standards for Diabetes Self-Management and VA/DoD Clinical Practice
Guidelines as a benchmark for program development. Utilizes Deming’s analytical
framework Plan-Do-Study-Act (PDSA) to further improve, measure, and benchmark
the programs at CAVHCS.
Trained facilitator for: Patient Education: TEACH for Success course. The VHA
healthcare transformation initiative to enhance PACT teams patient-centered
communication, health education and health coaching skills. FY11 and FY 12 VHA
goals met.
Championed implementation of Shared Medical Appointments (SMAs) which
included developing protocols and training staff. CAVHCS now has 10 ongoing
SMAs clinics including diabetes, hypertension, heart failure and pain. A1C’s have
improved 1.4%. other outcomes monitored LDL cholesterol, Framingham scores and
healthcare behavior goals
Instituted electronic consults for DSME and endocrine service line to improve
veterans access across the healthcare system.
Initiated Clinical Video Telehealth for Diabetes Self-Management Education
(DSME). Completed needs assessment, business plan and service agreement prior to
program initition. This accomplished PACT goal of offering educational services in
at least 2 outpatient clinics via Telehealth.

resulted from decrease need for fee basis referrals. Evidenced- based protocols were
implemented for the diabetes clinic in accordance with the American Diabetes
Association and VA/DoD clinical practice guidelines.
Diabetes Performance Measure Champion and implemented action plans to improve
clinical outcomes. Measurable improvements made in diabetes performance measures
(B/P control, A1C, lipid, and monofilament sensory foot exams) to fully successful or
exceptional. Served as a clinical resource for educational support for providers and
nursing staff.
Initiated and led a quality improvement process team for Smoking Cessation.
Outcomes related to the project included a more effective method of providing
prescription support for smoking cessation that improved resource utilization by
decreasing unscheduled walk-in visits to the primary care providers for

pharmacologic agents. Performance measure in smoking cessation increased from 92
to 99% and then remained above the benchmark.
