Objective I successfully accomplished high standard training in Adult
special cardiovascular Medicine at Case Western University,
interest and University Hospital's of Cleveland and Brigham and Women's
future plans hospital, Harvard Medical school, USA. I started my
profession in Cardiology in 11/1999, after completing
Internal medicine requirement at King Abdulaziz medical city,
National Guard Hospital in Jeddah, KSA. Pursuing suitable

Medical
Training 1-Fellow in Cardiac Intensive Care Unit ; 4 months rotation.
The cardiac fellow in University Hospital's of Cleveland
serves as the admission and administrative physician
responsible for all the ongoing operational aspects of the
CICU. The CICU is an 18 bed Unit with nearly 100% occupancy.
Daily rounds are made in the CICU with the attending
cardiologist, medical students and four assigned house
staff. The discussion on these rounds stresses the patient's
history, diagnostic physical findings, analysis of
electrocardiographic and angiographic studies, and review of
the patient's laboratory data. Plans are developed for
further diagnostic studies and decisions are made as to the
therapeutic approach for each patient. Discharge planning
begins soon after admission to the unit. The cardiac fellow
is responsible for the insertion of Swan-Ganz monitoring
catheters, arterial line placement and temporary transvenous
pacer insertion .Central lines usually inserted by house
staff under supervision of the fellow.
2- Fellow in Cardiac Consultation Service; 4 months
rotation.
The Consultation Service at UH provides cardiovascular
consultations to medical and surgical patients with
diagnostic and/or therapeutic problems. Average number of
consultations is 7-12/dayThe service also provides follow-up
care at the request of the attending physician, and it often
serves as the interface between the medical services and the
invasive and non-invasive laboratories.
The Cardiothoracic Surgery Service provides the cardiology
fellow with the opportunity to follow patients who are
undergoing cardiovascular surgery including CABG, valve and
transplantation. Patients are evaluated and managed in the
Surgical Intensive Care Unit and on the Cardiac Surgical
floor. The fellow is supervised by an attending
cardiologist. This service offers an excellent experience in
aggressive intensive care.
3-Fellow in Cardiac Non-Invasive Laboratories; 4 months
rotation in Echocardiography Lab, 1 months in Nuclear
Cardiology Lab.
The non-invasive laboratories perform echocardiography,
Doppler and color Doppler studies approximately 25 studies/
day. Transesophageal echo minimum 4 studies /day. Treadmill
exercise testing minimum 4/day. Stress echo and dobutamine
Echo minimum 4/day. Diagnostic radioisotope scans including
SPECT MPI, treadmill stress and pharmacological stress (
Adenosin Persantin and Dobutamine ) minimum 15/day.Viability
Thalium and pet scan studies when indicated. During the
Echocardiography and exercise/nuclear rotations, the
cardiology fellow obtains hands-on experience in performing
these studies and interpreting the graphic information.
4-Fellow in Cardiac Catheterization Laboratory; 4 months
rotation.
This rotation it UH provides an extensive experience, daily
average cases performed in the cath lab ranges between
10-20. For general cardiology fellows, the hemodynamic of
normal and abnormal circulatory states are stressed. Normal
and abnormal coronary anatomy as well as congenital coronary
indications and complications of diagnostic and therapeutic
techniques are reviewed. The performance of these procedures
is learned on a one-to-one basis with a faculty attending.
The techniques of diagnostic cardiac catheterization
including coronary and ventricular angiography, and
aortography are learned. Under supervision, the fellows are
responsible for the performance of the catheterizations, the
interpretations of the data and the presentation of the data
to members of the health care team.
5-Fellow in Electrophysiology Laboratory and service; 4
months rotation.
Fellows at UH rotate through the clinical Electrophysiology
Laboratory and Electrophysiology Clinical Service. Initial
emphasis is on recognizing and learning management of
arrhythmias .EP laboratory runs a busy pacemaker,
defibrillator and CRT implantation service. The
electrophysiology service includes active in-hospital
consultation functions (minimum of 7 consultations/day) and
an outpatient clinic with Pacer interrogation.
6- Fellow in Heart Failure and Transplantation Service; 2
months rotation.
The fellow at UH will participate in the daily clinical
rounds of the Heart Failure and Transplantation Service.
During the rotation, the fellow learns all aspects of
transplantation, including pre-operative selection,
peri-operative medical and surgical problems,
immunosuppression, and long term follow-up. Observation
during the transplant procedure itself is encouraged. In
addition, the management of end-state congestive heart
failure will be stressed, intravenous inotropic therapy,
intra-aortic counterpulsation and ventricular assist devices.
7-Fellow in Pediatric/Adult Congenital Heart Disease; 1 month
rotation.
One month at UH is spent rotating with the pediatric
Cardiologist learning congenital heart disease, especially
as it applies to the adult patient. Particularly attending
and reading with pediatric attending.
8- Oncall Cardiology fellow( 1 in 4 nights);
second on-call in emergency room evaluation of patients with
cardiac disease as, CICU,in patients consultations, and
performing emergency catheterization with attending on-call.
9- Out patient Clinic;
A weekly out patient clinic is operated by cardiology
fellows, an average 7- 10 patients seen by each fellow. Under
supervision of a senior Cardiology attending.
Cardiology Fellows Program at UH hosts a number of weekly
conferences;
1-Cardiology fellows clinical case conference;
Clinical Cardiology Fellows are responsible for weekly Fellow
clinical conferences. The format for these conferences is
case-based presentations by the clinical fellows (usually of
an "unknown" patient case), facilitated by faculty mentors.
2-Cardiology grand rounds;
Cardiovascular Grand Rounds are a Divisional Conference held
weekly where UH faculty and Visiting professors present the
latest in clinical, translational and basic science
research.
3- Cardiology core conference;
Weekly conferences designed to teach core concepts for
clinical Cardiology fellows.
4- Echocardiography weekly conference;
Weekly conference is organized by the Echocardiography
Laboratory, and encompasses fundamental and clinical issues
as well as clinical cases in echocardiography.
5- Cardiac Catheterization Laboratory weekly Conference.
6-Fellows weekly conference;
Supervised by a senior attending, various clinical cases
encountered by fellows during the week are discussed.
7- Weekly ECG conference.
Clinical experience at BWH, Harvard medical school.
6/2002-5/2004.
1-One year fellow ship in the Non-Invasive Echo
lab(5/2002-2/2003 then 3/2004-5/2004).
Preformed 5 Echocardiograms daily, and interpreted over 15
Echo's daily with senior attending readers. Preformed over
500 TEE's in the Echo lab and EP lab. Supervised Stress and
Dobutamine Echo's average 5/day. Activities included weekly
non-invasive imaging conference, presenting Echo's and
interacting with others. During this period I had the chance
to attend Cardiac MRI sessions for 3 months consecutively.
2- Nuclear Cardiology training for 6 months ( 3/2003-8/2003).
Duties include supervising treadmill stress, and
pharmacological stress, including Adensoin, Persantin and
Dobutamine . Reading Nuclear MPI, MIBI and Thalium,over 15
studies daily with senior Nuclear Cardiologists. Viability
studies on demand including thallium and Pet scan,
approximately 2-3 daily. Activities included weekly Nuclear
cardiology conferences. Fellows present clinical cases and
difficult cases are discussed with attending.
3-Rotation with Lown cardiovascular group for 6 months
(9/2003-2/2004).
The Lown is a clinical group attached to BWH affiliated with
Harvard University, I had the privilege to work with Dr, Lown
and his group. Caring for out patient and in-patient service.
Reading and performing Echo's, stress Echo's and involved in
out patient pacer interrogation. Additional activities
involved attending daily conferences at BWH including
Cardiology grand rounds, mortality reviews. fellow's Ecg and
clinical teaching sessions. In addition to non-invasive
weekly conference.
