Case Management
Follow all confidentiality laws related to client
information; maintain client charts systematically and
maintain after-care files; assist intervention/ drug screen
clients on as needed basis; conduct client intakes on as
needed basis; interview individuals to assess social,
educational, and emotional capabilities and assist clients
with work plans in relations to compatibility; assist
clients with career development through job search
strategies and select training program resources
appropriate to client goals, levels of development, needs,
capacities, and interests; assist individuals through group
and individual experiences to develop attitudes and social
skills for improved career responsibilities; review service
plan and perform follow-up to determine quantity and
quality of service provided to clients and status of cases;
compile records and prepare weekly case reviews; discharge
/terminate clients and place progress notes in charts on
weekly basis; demonstrate high level of commitment, loyalty
and dedication to perform job responsibilities.
Counseling/Facilitation
Evaluate needs of clients in educational and treatment
setting; facilitate educational sessions on as need
basis and chart progress notes; select methods and
curriculum best suited to implement educational programs
for specific audiences; incorporate recovery dynamics and
relapse prevention in curriculum;
create environment conducive to clients attaining maximum
achievements;
provide individual and group guidance services relative to
clients' education and needs; suggest and present ideals or
corrective actions to assist clients in making better
adjustments and planning intelligent life goals; counsel
individuals or groups regarding psychological or emotional
problems, such as stress, anger management, domestic
violence, conflict resolution, self-esteem, substance
abuse, character development, family situation, using
evaluative techniques and implement therapeutic treatment
plans in educational and therapeutic setting; interview
clients to obtain relative information; select and
administer various assessments, personality inventories,
and educational tests; conduct relaxation exercises and
peer counseling groups; refer clients to supportive
services to supplement treatment and counseling.
