CLERICAL:
• Greet all who enter the office and insure their needs are meet.
• Prepare Caregivers billing for submittal and email to Home Office disbursement center.
• Insure missed visits and missed tasks are included in the transmittal.
• Complete in-house notes for the RN and Operations Manager.
• Complete client updates as required.
• Insure all faxes are sent and received.
• Complete files as required or assigned by the Office Operations Manager.
• Log and distribute all in-coming mail and see that all out-going mail goes out on time.
• Assist the Care Coordinator as required.
• Received all referral calls.
• Up keep Supervisory Reports. Make sure the RN is making visits every sixty two days.
CARE COORDINATOR:
• Ensure that all clients are filled (#1 priority on a daily basis)
• Ensure that care is being provided according to all relevant policies, procedures, and
regulations
• Pre-interview applicants (at time of application) with no discriminating notes
• Conduct orientation with assistance from the RN and ensuring that ALL TAX FORMS
ARE FILLED OUT!
• Ensure that ALL caregivers meet all the requirements to provide in home care to the
elderly and disabled (CNA/PCA, Background check, Prior employment, etc.)
• Identify client needs
• Schedule work assignments with regard to caregiver qualifications and experience (New
clients, Fill- ins, Week end list, etc.)
• Discuss any issues or concerns with RN
• Keep RN informed of daily operations as needed
• Maintain current, accurate, and confidential client/caregiver reporting system to
Corporate Office and Audit Department (discrepancy report and summary, billing, new
clients/caregivers, updated caregiver information, trip files, etc.)
• Ensure that Corporate office is notified immediately of the death or loss of a client
• Ensure that Sympathy, Thank You, and Birthday cards are sent out to the appropriate
people
• Ensure that ALL reports are into Corporate office each week by the deadline (most are
Monday by 11am)
• Maintain ALL client files
• Maintain ALL accurate and confidential 9511 reports in client file – Also ensure that they
are distributed to DHS, Audit Department, and Case Managers (TCM/CSM)
• Ensure ALL forms on the Employee Check List is in the new hire packet and sent to HR
• Maintain current, accurate, and confidential caregiver files for each caregiver
• Maintain current, accurate, and confidential client complaint book (Care Coordinator
documents complaint and RN documents resolution)
• Maintain respite/ACS log on each client (if applicable)
• Maintain current and accurate active/non-active caregiver list
Active caregiver list – caregiver name and date of employment
Non-active caregiver list – caregiver name, date of employment, date of
discharge, immediately notify Human Resource, by email, of caregivers who have
quit, are discharged, or who have went from Full Time to Part Time
• Maintain and update caregiver schedules
• Assists RN in caregiver evaluations
• Knowledge of Excel Spreadsheet software, Microsoft Word and Outlook,
Proficient in use of email and internet and accurate keyboard skills
