My Name is Scott Swaby. I reside in Linton, IN. I am a licensed
Health Facility Administrator and a Registered Nurse. I have been an
Administrator since October 1, 2008 and a Registered Nurse since July of
1995.
I believe it takes a great team to make a building successful. There
are no one man operations in Long Term Care. I believe in going to great
lengths to retain employees. All employees struggle at some point in there
career. Once you sit down with an employee, that is struggling, and work
out the situation they tend to become very loyal employees. I believe in a
calm and fair workplace. Anything else becomes a distraction to Resident
Care. I believe in quality care. This also builds loyalty among current
Residents and their families who may be repeat customers.
I have had some recent successes. I worked at Glenburn Home in Linton
IN from May of 2010 to March of 2014. This was a building who did not have
a great survey history. The first year we received two tags that were paper
compliance. The second year we received a few tags but were compliant with
the first revisit. Then in 2013, we received a deficiency free survey. The
first one in 30 years at Glenburn. In 2010, Glenburn had a report card
score of 225 and currently the score is 70. I hired one of my surveyors as
my Director of Nursing and a second surveyor as my nursing consultant. I
assisted my Executive Director in starting an IGT program with Greene
County General Hospital.
I would certainly entertain the thought of relocation for the right
company.

I was a Health Facility Administrator at Glenburn Home. Glenburn Home
is a 137 bed facility
with a 34 bed secured Alzheimer's Unit, 10 beds dedicated to Medicare
only and 127
dually certified. Our medicare census ran from about 15 to 25. Our
Alzheimer's Unit was always
full. I was over the Long Term portion of Glenburn Home. Glenburn
Home also serves it
community with 26 independent duplexes and meals on wheels for the
organization known
as Generations.

I was employed at Garden Villa of Bedford. I started as a RN in the
Assistant Director of Nursing role with the intention of obtaining my HFA
license. My AIT was completed at Garden Villa under the Direction of
Tammy Keenan and I became HFA on October 1st 2008. I left Garden Villa
to work at Glenburn Home. I was Driving 100 miles per day at Garden Villa
and Glenburn was 5 blocks from my house. Garden Villa Bedford is a 140 +
bed facility with a secured Alzheimer's Unit, a Pediatric Unit, and
Skilled and NF beds.

I had an emergency surgery in November and was released to go back to

had resigned my position at Garden Villa Bloomington in late November
and while traveling
home my last day became ill. I had a ruptured bowel that had to be
surgically repaired. Wabash
Valley had hired me in November of 2005. I added this to show my gap
in work history.

I worked at Garden Villa Bloomington as a RN. I worked as a Staff
Development Coordinator,
scheduler, Assistant Director of Nursing, Director of Nursing, and I
finished as weekend
supervisor. Garden Villa Bloomington has 224 beds with a secured
Alzheimer's unit, a skilled
rehab unit, and NF beds. I left here due to feeling burned out. I had
done scheduling for 3 years
and worked as an Assistant Director of Nursing at the same time.

I worked for the Terre Haute Penitentiary (federal prison). I worked
as a RN passing
medications and doing assessments. I left here because I did not like
the Federal System and
because, at times, I would be the only health care professional on
the property. There was a
camp, a secured lock down, general population, and death row to take
care of. It was too much
for one person.

while getting my Associates of Science for my RN. I worked some as a
Graduate nurse and very
little as a Nurse due to no openings. I left here to pursue a full
