and worked as a staff pharmacist at Rite Aid for approximately one and a half
years. Started out in Brunswick, Maine and moved to a much busier store
w ith more demand for immunizations in Lisbon Falls, Maine. Also covered a
lot of stores throughout Maine when coverage was needed. After that,
worked as a clinical pharmacist generalist at Eastern Maine Medical Center,
a comprehensive 411-bed teaching hospital for almost four years. Worked
both as a central and decentralized pharmacist, with duties including order
p rocessing and review, pharmacokinetic dosing of antibiotics, with all
vancomycin mandated to be pharmacy to dose per hospital policy, oral and
parenteral anticoagulation dosing and monitoring, collaboration with
p rovider staff to optimize medication therapy, nursing staff support and
education, technician supervision and precepting of students. Also provided
p harmacist coverage for critical access hospitals within our hospital system.
E xtensive t raining and experience in the preparation of IV medication in a
797 compliant facility. Knowledge of IV drug compatibilities, stabilities, and
s terile preparation. Trained in chemotherapy preparation. Able to operate
Pyxis 3500 system including filling, dispensing, maintenance and
t roubleshooting. Experience with Talyst dispensing system. Well acclimated
to busy pharmacy environment and able to multi task.

• E ducate prescribers on available t reatment options for patients with
H IT
• Monitor patients being t reated for H IT, specifically monitoring our
formulary drugs argatroban and bivalrudin
Participate in pharmacist directed anemia management/ESA dosing for
outpatients
Outpatient consults for patients scheduled for bariat ric surgery. A
medication list is obtained and then formal recommendations are made to out
bariat ric surgical team about how to optimize medication post-operatively.
