physicians (office and hospital) and Kaiser Permanente personnel regarding Vitas, its purpose,
services provided during admission and care thereafter. Acted as additional source of
information to family as needed after admission and assisted case management staff with
special needs of patient and family members.

Patients referred by physicians for a palliative care consult were provided with the holistic,
interdisciplinary approach of the palliative care team to assess their need for palliative care
