Objective
To obtain employment in a position with potential for growth with a company that will allow me to apply my
education, skills and experiences for the benefit of the population served. I desire to work under a licensed
mental health professional who is authorized to verify the hours I work as well as the quality of my performance
which will be submitted to the Utah State Division of Occupational and Professional Licensing. I have career
goals that include becoming a SUDC. Employment in the substance abuse treatment field is the environment that

Professional Accomplishments
Mental Health Worker
Observe and report behavior of clients with anti-social conduct disorders.
Through supportive listening, prompting, encouraging, and crisis intervention techniques aided in carrying out
the care plan specific to each client as designed by the care team (therapist, doctors).
Was often required to intervene in physical confrontations.
Case Management (Certified Adult Mental Health Case Manager)
Observe and report behavior of clients with severe and persistent mental illness and homelessness issues. Many
have poly-substance abuse issues as well.
Through supportive listening, prompting, encouraging, and crisis intervention techniques aided in carrying out
the care plan specific to each client as designed by the care team (therapist, psychologists, case managers).
Billed Medicaid according to services provided one on one.
Facilities Custodial
Full-Time Custodian on the Support Crew for three and a half years.
As Sr. Custodian, I developed an efficient working knowledge of various processes and techniques specific to
the demands of several buildings.
Consistently scored in the nineties on IPEs and carried out responsibilities typical of a lead when absent.
Substitute Teacher
Maintained order and lead classroom activities as outlined by the teacher during his/her absence.
Taught lessons to children ranging in ages from 4th grade through high school.
Worked with students housed in the detention facility of the youth justice system.
Customer Service and Remote Computer Repair
Assist customers over the phone with troubleshooting and determining the fix/repair needed for their computer.
Placed orders for parts or service calls as needed.
