Case Manager for the Adult Medical Access Clinic
. Coordinated care for adult patients with chronic medical conditions
including but not limited to diabetes, hypertension and Hepatitis C.
. In cooperation with a social work technician started the Prescription
Assistance Program in Whitfield County.
. Implemented a Diabetes Management Class for 20 Medical Access Clinic
patients who had poorly controlled blood sugars. Invited physicians
in the specialties of nephrology, cardiology, ophthalmology, podiatry
to speak. Taught diet and exercise and it's effect on blood sugars.
Taught blood sugar checks and insulin administration.
. Taught patients about the effects of hypertension on the blood
vessels of the kidney, heart and brain.
. Provided blood pressure monitoring.
. Searched for physicians to take patients whose problems were outside
of the ability of the Health Department to adequately care for.
2003-2004 Whitfield County Health Department
Case Manager for the HIV Clinic
. Provided HIV testing and prevention counseling .
. Allocated Ryan White Funds to HIV+ patients according to strict
federal guidelines. The funds were used for housing, medication,
nutrition and referrals to physicians outside of the HIV clinic.

Schools, and Eastside Elementry.
. Duties include immunization audits, hearing and vision screenings,

have allergic reactions, diabetes, seizures or asthma.

I am currently unemployed and ready to work. I feel like nursing is my
