OPERATIONAL EXPERIENCE
ARMED FORCES OF THE PHILIPPINES MEDICAL CENTER is a military
training hospital with 1.200 bed capacity. This tertiary level hospital
caters to military personnel, their dependents and authorized civilians. It
provides medical; surgical; psychiatric and rehabilitation facilities for
the diagnostic treatment and disposition of all patients and provide
training in the different specialties under the different department such
as Affiliate Doctor's Residents; Post Graduate Medical Interns; Nursing
Students; Medical Technologists; On The Job Training Nurses, Physical
Therapists, Psychologists, and other Health Care Assistants.
The NEUROSURGERY WARD is composed of fifty seven (57) bed
capacity a that handles an emergency general medicine which provides
specialized diagnostic procedures, operative interventions and other forms
of treatments with different specialties specific to Neurosurgery.
The OUT PATIENT DEPARTMENT and EMERGENCY cases which cater
soldiers and their dependents are given follow up treatment as well as
diagnostic procedures like and emergency as needed specially dealing with
paramedicine.
JOB DESCRIPTION:
. Orients the patient to the ward environment and interpret
policies and routines
. Gives medicine orally and intravenously as ordered by the
attending physician
. Explains the procedure to be done to allay patient's
anxiety
. Check informed consent if any procedure to be done
. Obtain information about the patient's understanding of the
medical and surgical procedure in general terms
. Encourage the patient and family to discuss feeling or
anxieties regarding the surgical procedure and anticipated
results
. Comfort the patient, convey a sense of security and trust
(Touch as appropriate)
. Give health teaching prior to discharge and necessary
follow up (strict compliance to treatment regimen, proper
diet and rest, deep breathing and coughing exercises, early
ambulation, increase fluid intake and right diet)
DURING BRONCHOSCOPY AND THORACENTESIS
. Maintaining aseptic technique, sterility of packs,
instruments and supplies needed for every bronchoscopy and
therapeutic thoracentesis
. Assists the Medical Officer and other member of the
medical team in gowning, gloving and in draping the
patient aseptically
. During the procedure always one step ahead with high
anticipatory instinct of the doctor's needs during the
entire procedure
. Correct initial and final count of sharps, sponges and
instruments.
. After care of packs, instruments and equipments used
during surgery
. Assist the patient to the ward with proper endorsement(
like NPO for 4 hours and flat on bed, may observe for any
signs of respiratory distress)
Prior to surgery (if any referral to surgical operation)
certain preparations are prepared:
. Greet and identify the patient (check patients
identification, verify allergies or chemical sensitivities)
. Check plan of care and patient's chart for pertinent
information including signed consent
. Prevent dissemination of microorganism by wearing of N95
mask, covering patient's hair with cap, protecting linen
from being soiled
. Assist the patient to move onto the operating table taking
care of the patient's gown, blanket, intravenous (IV)
infusion tubing, catheters or drainage tubing using good
body mechanics to prevent self injury
. Maintain a safe OR environment for the patient (applying
safety straps, staying with the patient in the OR Suite)
. Prepare and label specimens for examination to laboratory
for accurate diagnosis
. Complete the patient's chart, operative records, charges
and laboratory requisitions
. Assist the patient/surgeon in proper positioning in
accordance with the surgical procedure to be done and
endorse properly to the Operating Nurse on duty of
pertinent assessment, treatment and procedure had done
Remind and instruct the patient with the post operative
health teachings e.g. early ambulation, deep
breathing/coughing exercise, abdominal guarding
DUTIES AND RESPONSIBILITIES
. Renders professional nursing care to patients (soldiers and their
dependents) of varied conditions complementary to medical regimen to
pursuant to the goals and objectives of hospital patient care;
. Perform nursing modalities for the comfort and well-being of the
patients;
. Administers prescribed medications, oral and injections;
. Observes patients reaction to medication and treatments;
. Observes progress of intravenous infiltration;
. Take temperature, pulse, respiration, heart rate and blood pressure
to detect deviations from nursing care plan;
. Observes, records and reports to supervisor or physician patients
condition and reaction to drugs, treatment and significant
incidents;
. Maintains patients medical records on nursing observations and
actions taken such as medications and treatments given reaction,
test intake and emission of liquid and solids, temperature pulse and
responsibilities rate;
. Records nursing needs of patients of nursing care plan to ensure
continuity of care;
. Observes emotional stability of patients expresses, interest in
their progress and prepares them for continuing care after
discharge;
. Explain procedures and treatments ordered to gain cooperation and
allay apprehension;
. Rotates on day, evening and night tours of duty and may go on on-
call duties when required;
. Perform independent, interdependent and dependent nursing functions

1. Augments the afternoon and the night shifts when abnormally busy. In
extreme emergencies the supervisor on duty may call all available
military nurses to include the NOD to augment any or all shifts.
2. Ensures Nursing Service Personnel who are sick in quarters (Nurses
Officers Quarters) receives appropriate medical and nursing
management.
3. Assists Supervisor on duty during mass evacuation at emergency room.
4. Accounts NC Officers during Red Alert and submit list to Supervisor on
Duty.
5. Communicates unusual and necessary activities of the day to the Chief
Nursing Service
MEDICAL TEAM/ DISASTER TEAM
Is a designation given to Nurse Corps Officer who goes on 24 hour from

> Medical Team for Medical and Dental Civic Action Program
(12-17 July 2008)
Dispatched as medical team for the Medical and Dental Civic Action
Program deployed in Capaclan, Romblon, Romblon, Philippines due to the
typhoon "FRANK" Captain Batalla, Capt Narag, Captain Catangay, 2Lt Agtoto,
Tsg Legarse Phil Army and PO2 Bundalian were among my companion during the
medical mission. Imbued with deep sense of commitment and professional
competence and dedication to duty, we had discharged our duties and
responsibilities as manifested in the efficient delivery of services in
performing immediate medical management for typhoon victims in Romblon.
During the same period, the tem was able to identify patients positive for
typhoid fever and gave them necessary treatment. Relatedly, the team
continued the conduct of Medical and Dental Civic Action to address
lingering health concerns By these mission, we were able to earned distinct
knowledge and skills not only for our organization but as well as for the
whole Armed Forces of the Philippines.

2Lt Reyes, 2Lt Perdido were my companions displayed wholehearted support
that led to the efficient management of the special occasion wherein the
center were able to celebrate the foundation anniversary more meaningful
and entertained their guests, patients and organic personnel and their
dependents to their fullest satisfaction. Indeed, the synchronization and
excellent execution of the whole activity were clear manifestation of
competence and initiative in the performance of their assigned tasks. By
this achievement, earned distinct credit not only for our organization but
as well as for the whole Armed Forces of the Philippines.

COURSE REPORTS/QUALIFICATIONS GAINED
> Philippine Military Academy Class 2005
(01 April 2001- June 2003)
The training indoctrinated the basic military knowledge as a cadet and
as an officer. During the course of the training, basic weapons training
and drills on marching and rifle handling were incorporated during the
first year. Some military culture and traditions are also taught as well as
the Military Courtesy and Discipline as the essential aspect of molding
into a good leader. Field Training Exercises are also conducted during the
training as the practical exercises in engaging through terrains and
obstacles in a given objective. Saturday inspection of barracks and
uniforms is also a crucial part for us not to have demerits.
Physical and Academic competencies are included as part of the grading
system. Mostly, over pressured compliances and compressed schedule as a
cadet, develops each cadet a perseverance, good attitude and character.
Usual routines in the training were part of our activities like reveille
road run in the morning and cadence in morning mess and directly go for
classroom instruction. There were also part of the training we had
individual combat training using techniques and skills in martial arts
which will be useful whenever encountered in actual combat. Having a squad
mate is also a part of the course wherein each upperclassman has
subordinate to handle. This is crucial in developing decision making
especially in times of difficulty. The course is solely to develop and
train each cadet to possess the character and the attitude to be a future
officer and a good leader who has perseverance and always willing to serve
the people.
> Nurse Corps Officer Probationary Training Course Class 01-07
(16 April- 08 November 2007)
The course had 6 months military training for nurses who will be
aiming for military nurse as an officer. Academic and non-academic
competition is also a part of the training, thus priority for those who
excel in the training will be rewarded. First month were focused more on
orientation to military and basic knowledge in the military like drills on
marching and familiarization with the firearms. Routine activities like
reveille road run every morning is always their usual early morning alarm.
Saturday inspection of barracks and uniforms is also a crucial part for us
not to have demerits. Proper uniform and bearing is also indoctrinated as
well as before going to the ward for duty.
Individual life saving situations like mass casualty of wounded
soldier or buddy is also a practical exercises Carrying each classmates
like fireman's carry and using lither wherein learning how to evacuate
emergency life threatening situations from which we save lives of a comrade
in arms. Several patients who are extremely injured are given particular
triage so that we could save and prioritize who are capable of saving time
and emergent action to save a life.
During the training, I was rotated in Thoraco-vascular ward, emergency
room, obstetrics and gynecology ward, surgical ward, oncology and medicine
ward, pediatric ward. Every rotation had an evaluation report of the
performance and attitude in the ward or duty. We had catered 60 patients
per ward wherein, learning and skill wise is very conducive and productive
at the same time.
Medical teams and disaster dispatch is also a part of our duty which
we were detailed for 24 hour alert/ on call for any medical missions and
deployment if any natural calamity and disaster. Compose of doctor and
medical aid man, together we report to designated area upon the release of
the order from operations.
I also gained basic life support seminar, basic intravenous therapy
and some seminars in the hospital and outside the hospital which are
helpful in the combat area and as well as in the community. Upon graduation
we were commissioned already, then, I gained points and merits that made me
number one(1) out of fifty-seven (57) nurses both in academics and physical
tests. I also received leadership award being as the Class President and
Physical proficiency award "Tarzan". The whole course of the training had
gained us more knowledge and skills in nursing as a military nurse which
could be our weapon in any situations may arise. Military nurse in the
Philippines is challenging job because of some areas in the country are
Islamic community where we integrate and know some of their culture and
traditions.
