Walter Reed National Military Medical Center, Bethesda, MD.

Serves as primary care clinical nurse RN team leader, responsible for

daily triaging of active duty military members on a sick call, and over the

phone . Presenting patients complaints and health care concern and

communicates triage decision and information to the appropriate health care

provider .Assesses physical and mental health status of patient on acute

care bases with documentation of data, triage decisions to the

appropriate providers and follow up with necessary referrals. Oversees

the medical home port team in updating patients data and utilizes

communications and management tools US DoD Military Health securing

messaging system. Tools . (i.e. AHLTA, CHCS-1,RelayHealth and Care

Point acts as a liaison nurse between patients and their providers on

relay health, enrolled patients on RelayHealth, medications refill, labs,

referrals .Booking patient appointments, daily huddles with supports staff

and fellow registered nurse. Scrub providers scheduled for patients easy

access to self booked appointments online. Scheduled patient's vasectomy
