I was responsible for all the clinical staff including HHA’s. Supervising, teaching of staff. In

charge of all the on-call. Over four clinical managers, utilization review, billing dept. I did all the

hiring, disciplinary action and letting go of staff. Assisted in State and Insurance Audits.

March of 2010 to June of 2011.

I oversaw all the care of my residents. Provided family meetings, dealt with family conflicts.

Supervised all staff, hiring, and firing. Medications. Made sure that all the State and local

regulations were followed. Passed a State Survey. Received my Basic Administrator Course

from UALA. Loved the residents.

Tooele branch and moved everything to SLC .Which they did with a majority of their offices

when they were bought out by Gentiva.

Opened up the Tooele County Area for Creekside Home Health Care and Hospice with two

branches. Was the only Nurse for 6 Months, until we grew. Hired Staff set-up new offices.

Provided all duties as assigned to a Branch Manager. Took on call duties and provided clinical

care to pts. In their home settings.
