CAREER PROFILE AND OBJECTIVES:

I offer my many years of experience to help create an environment that will

be beneficial to the residents and staff and to the service facility

owners. I have excellent relations with the residents and their families,

company staff, doctors, and pharmacy personnel. I have earned the respect

of my peers and subordinates and have a reputation for being tough but

fair. I am uncompromising when it comes to compassionate care for the

residents and will not tolerate abuse or neglect. I have strong

organizational skills. I know all my residents' names. I keep copies of all

laboratory reports, daily schedules, and head-count sheets in my office in

chronological order for easy access. I in-service the staff in accordance

with State guidelines.

Professional Experience

Aurora Rehabilitation & Living Center:

January 2013 - Recent

During my current tenure at Aurora Rehab as the DON I have established the

following:

< Walk-to-dine.

< Fall protocol and meetings to monitor the falls.

< Pneumonia and Flu consent forms were current.

< Removed side-rails from beds and replaced them with mobility devices.

< Helped to increase census by taking more medically complex residents.

The Inn at Willow Falls:

August 2012-October 2012

I was employed as the DON and quickly learned their data entry systems.

During this short period of time with the permission of the administrators:

< I established the use of an in-house laboratory.

< I arranged for in-house x-rays to be performed at the facility in

order to spare the residents and their families having to leave the

facility for these services.

< I also added sphygmomanometers and stethoscopes to the med-carts to

facilitate ready access.

Dolton Nursing and Rehab Center:

2011-2012

I was employed as a DON where I initiated the following changes and

accomplishments:

< I reduced the excessive overtime from 200 hours in a two week period

to less than 20 hours by adding staff.

< I initiated the Pneumonia Vaccine program making sure that all

residents were offered the vaccine and that it was administered in a

timely manner.

< I assisted in the Falling Leaves program and Infection Control,

inservicing the staff so they would be informed of the latest CDC

guidelines.

< I recently completed a 20-hour Wound Care class.

Moraine Court Retirement Community:

2008 - 2011 DON

I joined Moraine Court when they were converting to a Supportive Living

Facility. Working closely with the owners I created the forms and documents

necessary for the start up of the enterprise. I am very proud of receiving

five perfect State surveys including personal commendations from the State

on the achievement. The census grew from 50% to 90%.

Applewood Nursing & Rehabilitation Center, LLC

2006 - 2008

I was the Director of Nursing for this 115 bed facility. I managed a

skilled Medicare unit of 24 to 30 residents. I eliminated the need for

agency staff resulting in significant savings. I am skilled on the computer

and I instructed my nursing staff on data entry into Applewood's

computerized medical records system.

Brighton Gardens, Burr Ridge, IL:

2005 - 2006

I was the Health Services Director over 104 assisted living apartments and

a 30 bed Medicare unit. My knowledge of Medicare helped to facilitate the

Medicare unit. I insured that the Medicare unit is profitable by qualifying

potential admits and informed staff as to how to meet the Medicare

requirements.

I initiated Walk-to-dine and Bowel-and-bladder programs.

The facility previously had seven tags from the State before I came to the

facility. When the State made a surprise inspection in January 2006, we

received only two minor tags under my supervision.

Regal Healthcare & Rehabilitation Center:

2005

I was the Director of Nursing at this 120 bed facility with a very diverse

resident population. Residents consisted of mentally ill, geriatrics, and

ventilator dependant adults.

When I first began, they had thirteen tags from the State that were costing

them $500 per day in fines, which I cleared up within two weeks of

starting. I established programs, provided structure and leadership to the

staff and performed the tasks of hiring and scheduling.

Hickory Hills Nursing Home:

2004 -2005

I was the Director of Nursing at this 70 bed facility. The resident

population was composed of the mentally ill, geriatric, and Medicare

recipients. It was a challenging and career expanding position, where I

learned how to meet a broad spectrum of residents' needs. I established a

schedule for monthly summaries and established and maintained up-to-date

attendance sheets for all the nursing staff. I initiated policies for

Falling Stars and Walk to Dine at all meals for the residents. I developed

a written test for CNA applicants to determine their knowledge base and

assist in the hiring process.

My responsibilities included nursing employment evaluations and processing,

nursing staff scheduling, inservicing in accordance with State guidelines,

review of potential admits to ensure that we can meet their needs, ordering

nursing supplies, and evaluating incident reports for follow up at Quality

Assurance meetings.

Alden Nursing Homes:

2001 - 2004

I began as a floor nurse and accepted the position of Clinical Support

Supervisor in March 2002. I established a whirlpool and exercise program,

which increased department revenues from $900.00 per month to over

$3,000.00 per month. I was called upon to assist at other facilities to

prepare them for IOC's. I initiated the use of low-beds, in-serviced the

CNA's, served as on-call nurse manager, and assisted in the daily problem

solving as a department head.

I served as interim Director of Nurses, Sub-acute Director, and Assistant

Director of Nurses as the needs arose at various facilities.
