Job Description:
STAFF NURSE:
. Plans and gives good nursing care to the patients according
to their individual needs.
. Observes, records and reports symptoms and conditions of the
patients.
. Administers medications and carries out treatment and
procedures and notes reactions.
. Sponge baths and feeds moderately ill patients.
. Supervises the care given by the midwife, nursing aides and
orderlies in any team plan for nursing care.
. Assist in orientation and evaluation of personnel.
. Assist in patient's education and rehabilitation, including
promotion of mental and physical health.
. Assist physicians in the treatment or in administering
specialized therapy and in performing complicated
procedures.
. Substitutes for the Charge nurse when so delegated.
. Participates in the general activities of the nursing
services.
CHARGE NURSE:
A. Responsibilities towards Patients:
. Makes adequate provision for the nursing care of the patients
in the unit and supervise that care.
. Identifies nursing service problems needing care of patients
in the unit and supervises that care.
. Sees that physician's orders are promptly carried out and
properly checked.
. Provides for and supervises the administration of treatment
and medications ordered by medical staff.
. Provides for accurate descriptive records of the medical
treatment carried out by the nurses and the nursing care
given to the patients.
. Arranges for the assistance required by the members of the
medical staff and other professional personnel, i.e.
Pharmacist, dentist, dietitians, private duty nurses,
physical therapist, and coordinates with them in providing
for the patients total needs.
B. Responsibilities towards personnel:
. Assign specific duties of nursing personnel, coordinates and
supervises their activities.
. Evaluates and reports the quality of the services given by
the staff nurses, auxiliaries, and counseling them on basis
of findings.
. Assist in the orientation of the personnel in the unit.
C. Responsibilities towards the environment, equipment, supplies, etc.
. Directs housekeeping activities that are related with patient
care.
. Checks the equipment in the unit that is in good working
order. Facilitates the repair of out of order equipment.
D. Other related responsibilities:
. Utilizes all opportunities to enrich the clinical experience
afforded to the staff nurses and other personnel in the
operational level.
. Assist in the professional growth of all personnel and
provides learning opportunities for nursing and other
affiliates.
. Motivates personnel to actively participate in all
activities of the nursing service Department.
HEAD NURSE:
. Maintain general supervision of nursing care given to the patients and
all nursing activities within the Nursing unit.
. Analyzes and evaluate the kind and amount of nursing service required
in the nursing units, and in cooperation with the charge nurse, plan
for its effective administration in the units.
. Coordinates with the medical staff regarding patients care.
. Interprets the principles of management to the charge nurses, other
personnel and encourages them to apply these principles in their daily
work.
. Coordinates with the charge nurses regarding assignments of duties and
time table of personnel in each unit to facilitate efficiency,
effectivity and personnel satisfaction.
. Makes rounds in the nursing units to:
a. Ascertain quality of patient care.
b. Evaluate the environment for cleanliness, optimum comfort and
safety of the patient.
c. Determine needs for improving work environment for personnel.
. In cognizant of the legal aspects of nursing, demonstrating this in
her assignments, supervision and teaching.
. Participates in informal teaching of patients and their families and
assists personnel in developing teaching skills.
. Confers with the patient, patient's relative and visitors regarding
patient's illness, recovery and rehabilitation.
. Provides for meeting the spiritual needs of the patients.
. Reviews and helps updates Kardex, checks charts and nursing records
to see that orders have been promptly carried and appropriate has
been made.
. Plans periodic with in her staff for the purpose of improving nursing
care within the unit, and for the achievement of long and short term
goals established for the unit.
. Evaluates and records the quality of service given by the charge
nurses, and assist them in the evaluation of other nursing service
personnel and counseling them on the basis of findings.
. Continuously studies and evaluates new methods or rendering quality
patients care for the purpose of promoting continuity and
productivity.
. Plans for, and participates in the education, training and
orientation of nursing service personnel.
. Coordinates activities with other non- nursing department, solves
minor problems to her immediate supervisor and refer major recurrent
problems to the immediate supervisor.
. Participates actively in committee work and encourages her personnel
to do the same.
. Helps in investigating complaints or expression of dissatisfaction
regarding service rendered.
. Secures supplies and equipments; and oversee their use and care.
. Keeps the supervisor informed of the needs of the nursing units and
any special problems that may arise.
INTENSIVE CARE UNIT STAFF NURSE:
. Provides total, direct nursing care consistent with the plan of care
and needs of the patients with respect for the patient's privacy,
dignity, personal preference and culture.
. Demonstrate priority setting skills and timeliness in responding to
the patient needs.
. Transcribe physicians order and reviews nursing and medical orders for
accuracy and appropriateness.
. Prepares, administers and documents the administration of oral,
parenteral and topical medications as prescribe by the physicians. Is
knowledgeable of therapeutic effects, usual dosages, contraindication,
side effect and adverse reactions for all drugs and parenteral fluids
administered in.
. Intervenes appropriately on clinical emergency situation i.e
cardiopulmonary arrest, initiating emergency measures by:
1. Assessing the patient and initiating CPR, if indicated and
certified to do so.
2. Alerting subordinates to summon for help and call for CODE BLUE,
if necessary.
3. Informing the treating doctor to assess the patient for
emergency management.
4. Never to leave the patient all alone by him/himself.
. Perform wide range of ICU procedures/interventions with attention to
accuracy, patient safety and comfort, including those requiring strict
aseptic technique such as:
1. Preparation of ICU equipment, blood components and drugs to be
ready for any emergency.
2. Assist treating doctors during ICU procedures.
3. Monitor patient for progress of blood transfusion process and
record observation.
4. Anticipates needs that may arise during procedure i.e need for
cross matching blood, ABG, central line insertion etc.
5. Observe patient for possible complications.
6. Endorses to the nurse in charge if patient is to be admitted as
to the procedure done to patient, medical condition and
continuing care to be done.
. I.V therapy insertion maintenance/discontinuing per doctors order and
hospital policy
. Carries out doctor's order with accuracy and timely manner, clarifying
orders from the physician, if it is unclear and letting the physicians
to sign within 24 hours if it is verbal order.
. Calculate with the co-nurse for any dosage calculations of big risk
medications.
. Assessment of patient risk and application of appropriate precaution
against accidental fall.
. Administer blood and blood products per doctor's order and hospital
policy.
. Administer medication following the seven (7) R's of medications.
. Constantly monitors the patient for level of consciousness, vital
signs, oxygen saturation, during procedure documenting the same
appropriate nursing form where applicable.
. Assesses patient under restraint and provide privacy for:
1. Circulation checks to any limb that is restrained.
2. Response to the restraints and to report to the treating doctor and
adverse effects of restraints to the patient.
ENDOSCOPY UNIT:
. Provides total, direct nursing care consistent with the plan of care
and needs of the patients with respect for the patient's privacy,
dignity, personal preference and culture.
. Demonstrate priority setting skills and timeliness in responding to
the patient needs.
. Transcribe physicians order and medical orders for accuracy and
appropriateness.
. Obtain a signed consent before the procedure started.
. Prepares, administers and documents the administration of oral,
parenteral and topical medications as prescribe by the physicians. Is
knowledgeable of therapeutic effects, usual dosages, contraindication,
side effect and adverse reactions for all drugs and parenteral fluids
administered in.
. Sets up equipment and drugs to be ready for endoscopy/colonoscopy
procedures and assist physicians during diagnostic and therapeutic
procedure performance.
. Performs wide range of endoscopy/ colonoscopy procedures with
attention to accuracy, patient safety and comfort by observing these
steps:
-Prepare all equipments and drugs to be ready for the procedure.
- Checked carefully the endoscopy machine before the procedure
started.
-Monitor and record vital signs before and after the procedure.
-Constantly monitors the patient for level of consciousness, vital
signs, oxygen saturation during the procedure.
-I.V therapy insertion maintenance/discontinuing per doctors order.
-Observes patient for 1 to 2 hours if I.V sedation was given
before discharging the
patient.
